摘要
目的探讨大黄芒硝外敷腹部联合耳穴压豆在妇科恶性肿瘤腹式术后腹胀患者中的应用效果。方法选取2023年1月至12月江西省妇幼保健院收治的120例行妇科恶性肿瘤腹式手术术后腹胀患者作为研究对象,按照随机数字表法分为观察组与对照组,每组60例。对照组采用常规护理、心理疏导及健康宣教,观察组在对照组基础上采用大黄芒硝外敷腹部联合耳穴压豆干预,比较两组患者胃肠功能、腹胀恢复情况、中医症状积分及安全性。结果观察组干预后肠鸣音恢复、肛门排气、排便、腹胀缓解时间短于对照组,差异有统计学意义(P<0.05)。干预后,两组患者腹胀、腹痛、恶心、纳差中医症状积分及总分低于本组干预前,且观察组评分低于对照组,差异有统计学意义(P<0.05)。两组患者在两种方案干预期间均未出现严重不良反应,观察组仅有1例患者腹部皮肤轻微发红,未做特殊处理,两组患者安全性比较,差异无统计学意义(P>0.05)。结论大黄芒硝外敷腹部联合耳穴压豆应用于妇科恶性肿瘤腹式术后腹胀患者,可缩短胃肠功能恢复的时间,改善腹胀、腹痛等中医症状,且安全性高。
Objective To investigate the application effect of rheum officinale and mirabilite applied to abdomen combined with ear acupoint pressing bean in patients with abdominal distension after abdominal surgery of gynecological malignant tumor.Methods From January to December 2023,120 patients with abdominal distension after abdominal surgery of gynecological malignant tumor in Jiangxi Provincial Maternal and Child Health Hospital were selected as research objects,and they were divided into observation group and control group according to random number table method,with 60 cases in each group.The control group received routine nursing,psychological counseling and health education,and the observation group received rheum officinale and mirabilite applied to the abdomen combined with ear acupoint pressing bean intervention on the basis of the control group.The gastrointestinal function,abdominal distension recovery,traditional Chinese medicine symptom score and safety of the two groups were compared.Results After intervention,the recovery time of bowel sound,anal exhaust,defecation and abdominal distension in observation group were shorter than those in control group,and the differences were statistically significant(P<0.05).After intervention,the scores and total score of traditional Chinese medicine symptoms of abdominal distension,abdominal pain,nausea and poor appetite in the two groups were lower than those before intervention,and the scores in the observation group were lower than those in the control group,with statistical significances(P<0.05).There were no serious adverse reactions in the two groups during the intervention of the two programs,and only 1 patient in the observation group had slight redness of abdominal skin without special treatment,and there was no statistical significance in the safety comparison between the two groups(P>0.05).Conclusion The application of rheum officinale and mirabilite on abdomen combined with ear acupoint pressing bean in patients with abdominal distension after abdominal surgery of gynecological malignant tumor can shorten the time of gastrointestinal function recovery,improve the symptoms of abdominal distension,abdominal pain and other traditional Chinese medicine symptoms,with high safety.
