摘要
目的:探究血液灌流联合连续性血液净化治疗严重药物中毒患者的效果。方法:选取2022年3月—2024年2月赣州市赣县区人民医院收治的90例严重药物中毒患者为研究对象,将其随机分为研究组和对照组,各45例。对照组采用常规治疗,研究组在对照组基础上采用血液灌流联合连续性血液净化治疗,比较两组肝肾功能、临床指标、抢救情况、并发症发生情况及预后情况。结果:治疗后,两组血肌酐、尿素氮、天冬氨酸转氨酶和丙氨酸转氨酶均低于治疗前,且研究组均低于对照组,差异均有统计学意义(P<0.05)。研究组清醒时间、呼吸机脱机时间和代谢性酸中毒纠正时间均短于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为4.44%,低于对照组的17.76%,差异有统计学意义(P<0.05)。治疗3个月后,研究组预后情况优于对照组,差异有统计学意义(P<0.05)。研究组抢救成功率为95.56%,高于对照组的82.22%,差异有统计学意义(P<0.05)。结论:血液灌流联合连续性血液净化可以有效提高严重药物中毒患者的抢救成功率,在改善患者肝肾功能的同时,减少并发症的发生风险,改善预后,促进患者恢复。
Objective:To explore the efficacy of hemoperfusion combined with continuous blood purification in the treatment of patients with severe drug poisoning.Methods:A total of 90 patients with severe drug poisoning admitted to Ganzhou Ganxian District People's Hospital from March 2022 to February 2024 were selected as the study objects and randomly divided into a study group and a control group,with 45 cases in each group.The control group received conventional treatment,and the study group received hemoperfusion combined with continuous blood purification treatment on the basis of the control group.The liver and kidney function,clinical indexes,rescue conditions,occurrence of complications and prognosis of the two groups were compared.Results:After treatment,serum creatinine,urea nitrogen,aspartate aminotransferase and alanine aminotransferase in two groups were lower than before treatment,and the study group was lower than the control group,the differences were statistically significant(P<0.05).The waking time,ventilator offline time and metabolic acidosis correction time of the study group were shorter than those of the control group,the differences were statistically significant(P<0.05).The incidence of complications in the study group was 4.44%,which was lower than 17.76%in the control group,the difference was statistically significant(P<0.05).After 3 months of treatment,the prognosis of the study group was better than that of the control group,the difference was statistically significant(P<0.05).The success rate of rescue in the study group was 95.56%,which was higher than 82.22%in the control group,the difference was statistically significant(P<0.05).Conclusion:Hemoperfusion combined with continuous blood purification can effectively improve the rescue success rate of patients with severe drug poisoning,reduce the risk of complications,improve the prognosis and promote the recovery of patients while improving the function of liver and kidney.
