摘要
目的探讨子宫内膜癌(EC)组织中黏结合蛋白多糖-1(syndecan-1)和E-钙黏蛋白(E-cadherin)表达与临床病理特征及预后的关系。方法选取2019年7月至2022年12月海安市人民医院收治的60例EC患者作为EC组,选取同期于同院进行检查且经病理证实为非典型增生子宫内膜和正常子宫内膜组织的患者分别作为非典型增生子宫内膜组和正常组,每组30例。比较三组syndecan-1和E-cadherin表达情况、EC组不同临床病理特征syndecan-1和E-cadherin的表达差异,采用多因素Logistic回归分析EC组syndecan-1和E-cadherin表达的影响因素,采用Kaplan-Meier生存曲线分析syndecan-1和E-cadherin表达与患者预后的关系。结果EC组syndecan-1强阳性表达率高于非典型增生子宫内膜组和正常组,E-cadherin强阳性表达率低于非典型增生子宫内膜组和正常组(P<0.05)。EC组不同组织学分级、国际妇产科联盟(FIGO)分期及淋巴结转移情况患者syndecan-1和E-cadherin表达存在差异(P<0.05)。组织学分级、FIGO分期、淋巴结转移是EC组syndecan-1和E-cadherin表达的影响因素(P<0.05)。syndecan-1强阳性表达患者累积生存率显著低于弱阳性表达患者,E-cadherin强阳性表达患者累积生存率显著高于弱阳性表达患者(P<0.05)。结论EC组织中syndecan-1强阳性表达率较高,E-cadherin弱阳性表达率较高,两者表达均与EC临床病理特征及预后存在一定关联。
Objective To investigate the relationship between the expressions of syndecan-1 and E-cadherin in endometrial carcinoma(EC)tissues,clinicopathological features and prognosis of EC.Methods A total of sixty patients with EC who were admitted to Hai′an People′s Hospital from July 2019 to December 2022 were selected as EC group.Patients with pathologically confirmed atypical endometrial hyperplasia and those with normal endometrium who received examinations in the same hospital during the same period were selected as the atypical endometrial hyperplasia group and the normal group respectively,with 30 cases in each group.The expressions of syndecan-1 and E-cadherin was compared among the three groups.Differences in the expressions of syndecan-1 and E-cadherin between patients with different clinicopathological features in the EC group were analyzed.Multivariate Logistic regression analysis was performed to identify factors influencing the expressions of syndecan-1 and E-cadherin in the EC group.Kaplan-Meier survival curve was used to analyze the relationship between the expression of syndecan-1 and E-cadherin and the prognosis of patients.Results The strong positive expression rate of syndecan-1 in the EC group was higher than those in the atypical endometrial hyperplasia group and the normal group(P<0.05).The strong positive expression rate of E-cadherin was lower than those in the atypical endometrial hyperplasia group and the normal group(P<0.05).There were differences in the expressions of syndecan-1 and E-cadherin in EC group with different histological grades,different international federation of gynecology and obstetrics(FIGO)stages,and lymph node metastasis(P<0.05).Histological grading,FIGO staging,and lymph node metastasis were influencing factors of the syndecan-1 and E-cadherin expressions in the EC group(P<0.05).The cumulative survival rate of patients with strong positive expression of syndecan-1 was significantly lower than that of patients with weak positive expression,and the cumulative survival rate of patients with strong positive expression of E-cadherin was significantly higher than that of patients with weak positive expression(P<0.05).Conclusions The strong positive expression rate of syndecan-1 and weak positive expression rate of E-cadherin in EC tissues are high.There is certain correlation between the expressions of the two and the clinicopathological features as well as the prognosis of EC.
