摘要
目前结直肠癌(CRC)的治疗方式以根治性手术治疗为主,但由于手术中麻醉、创伤及术后消化道重建等多种因素,患者术后易出现腹痛腹胀、恶心呕吐、便秘等术后胃肠功能障碍(POGD)症状,不仅给患者及家属带来了经济和心理负担,还严重影响了患者术后康复。本文回顾近年来国内外相关文献发现,中医外治法以整体观念为基础结合辨证论治,现已在防治结直肠癌患者术后胃肠功能障碍方面得到广泛应用,且临床疗效显著,主要包括针刺疗法、艾灸疗法、耳穴疗法、穴位贴敷、中药热奄包等,可通过调节胃肠激素水平、炎症因子释放、5-HT信号系统、内源性阿片肽释放等机制促进CRC患者术后胃肠功能恢复,对提高结直肠癌患者术后康复以及生存质量具有重要意义。
However,due to various factors such as anesthesia,trauma and postoperative gastrointestinal reconstruction,patients are prone to postoperative gastrointestinal dysfunction(POGD)symptoms such as abdominal pain,bloating,nausea and vomiting,constipation,etc.,which not only brings economic and psychological burden to patients and their families,but also seriously affects the postoperative recovery of patients.This paper reviews the relevant literature at home and abroad in recent years,shows that the external treatment of traditional Chinese medicine is based on the holistic concept and combined with syndrome differentiation and treatment,and has been widely used in the prevention and treatment of postoperative gastrointestinal dysfunction in colorectal cancer patients,and the clinical effect is remarkable,mainly including acupuncture therapy,moxibustion therapy,auricular acupoint therapy,acupoint application,traditional Chinese medicine hot packing,etc.,which can promote the recovery of postoperative gastrointestinal function in CRC patients by regulating gastrointestinal hormone levels,inflammatory factor release,5-HT signaling system,endogenous opioid peptide release and other mechanisms.It is of great significance to improve the postoperative recovery and quality of life of colorectal cancer patients.
