摘要
目的探讨多层螺旋CT联合能谱CT对胃癌的诊断价值。方法选取152例疑似胃癌患者,所有患者均接受多层螺旋CT、能谱CT检查以及穿刺取材组织病理活检,以组织病理学检查作为金标准,分析多层螺旋CT联合能谱CT检查对胃癌的诊断效能。结果152例疑似胃癌患者经组织病理学确诊117例胃癌,占比76.97%(117/152)。以组织病理学作为金标准,多层螺旋CT检查对胃癌检出率为84.62%(99/117),能谱CT检出率为87.18%(102/117),联合检出率为94.87%(111/117)。联合检查的检出率高于多层螺旋CT、能谱CT单独检查的检出率,差异有统计学意义(P<0.05)。联合检查诊断胃癌的敏感度高于多层螺旋CT、能谱CT单独检查,差异有统计学意义(P<0.05),联合检查诊断胃癌的准确率和阴性预测值均高于多层螺旋CT,差异有统计学意义(P<0.05)。结论多层螺旋CT联合能谱CT可有效诊断胃癌,且与金标准有较好的一致性,临床应用价值较高。
Objective To explore the diagnostic value of multi-slice spiral(CT)combined with energy spectrum CT for gastric cancer.Methods A prospective study was conducted on 152 suspected gastric cancer patients admitted to the hospital from September 2021 to December 2023.All patients underwent multi-slice spiral CT,energy spectrum CT examination,and biopsy of tissue samples.The diagnostic efficacy of multi-slice spiral CT combined with energy spectrum CT for gastric cancer was analyzed using tissue pathology examination as the gold standard.Results Among 152 suspected gastric cancer patients,117 were diagnosed with gastric cancer through histopathology,accounting for 76.97%(117/152).Histopathology was used as the gold standard,the detection rate of gastric cancer in multi-slice spiral CT examination was 84.62%(99/117),the detection rate of energy spectrum CT was 87.18%(102/117),and the combined detection rate was 94.87%(111/117).The detection rate of joint examination was higher than that of multi-slice spiral CT and spectral CT alone examination(P<0.05).The sensitivity of joint examination in diagnosing gastric cancer was higher than that of multi-slice spiral CT and energy spectrum CT alone,with a statistical significant difference(P<0.05).The accuracy and negative predictive value of joint examination in diagnosing gastric cancer were higher than those of multi-slice spiral CT,with a statistical significant difference(P<0.05).Conclusion Multi-slice spiral CT combined with energy spectrum CT can effectively diagnose gastric cancer,and has good consistency with the gold standard,with high clinical application value.
