摘要
目的 分析EB病毒抗体联合核酸检测在儿童不明原因发热诊断中的意义.方法 选择常熟市支塘人民医院检验科2022年10月-2023年12月收治的124例儿童不明原因发热患儿为研究对象,应用不同检验方式将患儿分为对照组(n=62)和研究组(n=62),对照组患儿给予EB病毒抗体检测,研究组患儿给予EB病毒抗体联合EB病毒核酸(EBV-DNA)检测,分别对两组患儿的EB病毒抗体和联合检测的检测结果进行比较.结果 对照组EB病毒抗体阳性检出率为80.65%,研究组的EB病毒抗体联合EBV-DNA阳性检出率为87.10%,研究组联合检测的阳性率显著高(P<0.05);EB病毒抗体检测的敏感度为92.59%,特异度为66.67%,准确度为95.00%,阳性预测值为92.59%,阴性预测值为100.00%;EB病毒抗体联合核酸检测的敏感度为96.30%,特异度为75.00%,准确度为99.95%,阳性预测值为96.30%,阴性预测值为75.00%;EB病毒抗体联合核酸检测的敏感度、特异度以及准确度显著高于EB病毒抗体检测,阳性预测值显著高于EB病毒抗体检测(P<0.05).结论 将EB病毒抗体联合核酸检测应用于儿童不明原因发热诊断中具有较好的意义,联合诊断能够较好地反映出EB病毒感染之后的抗体水平,其本身具有较高的检测敏感度以及特异性,检验操作流程简便,可为临床儿童不明原因发热诊断提供借鉴.
Objective To analyze the significance of EB virus antibody combined with nucleic acid detection in the diagnosis of unexplained fever in children.Methods One hundred and twenty-four children with fever of unknown origin admitted to the clinical laboratory of our hospital from October 2022 to December 2023 were selected as the research objects,and the patients were divided into control group(n=62)and study group(n=62)by different test methods.The patients in the control group were given EB virus antibody test,and the patients in the study group were given EB virus antibody combined with EB virus nucleic acid(EBV-DNA)test.Results The positive rate of EB virus antibody in the control group was 80.65%,and the positive rate of EB virus antibody combined with EBV-DNA in the study group was 87.10%,and the positive rate of combined detection in the study group was significantly higher(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of EB virus antibody detection were 92.59%,66.67%,95.00%,92.59%and 100.00%,respectively.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of EB virus antibody combined with nucleic acid detection were 96.30%,75.00%,99.95%,96.30%and 75.00%,respectively.The sensitivity,specificity and accuracy of EB virus antibody combined with nucleic acid detection were significantly higher than that of EB virus antibody detection,and the positive predictive value was significantly higher than that of EB virus antibody detection(P<0.05).Conclusion It is of great significance to apply EB virus antibody combined with nucleic acid detection in the diagnosis of unexplained fever in children.The combined diagnosis can better reflect the antibody level after EB virus infection,and it has high detection sensitivity and specificity,and the inspection operation process is simple,which can provide reference for the clinical diagnosis ofunexplained feverinchildren.
