摘要
目的:探究幽门螺杆菌(Hp)阴性胃高级别上皮内瘤变(HG-IEN)的内镜及组织学特征。方法:回顾性抽取2020年3月至2023年4月陕西省核工业二一五医院收治的103例经外科手术或内镜黏膜下剥离术切除后病理诊断为Hp阴性胃HG-IEN患者的临床资料,统计并观察病变的内镜下表现及组织学特征。结果:103例患者中,病灶位置主要是胃窦(65例,占63.11%),病灶形态主要是隆起型(69例,占66.99%),病灶长径主要集中在0.5~0.9 cm(46例,占44.66%)、1.0~1.9 cm(38例,占36.89%),病灶外观主要是表面糜烂(35例,占33.98%)、表面结节(36例,占34.95%);病灶颜色主要是正常或发白(68例,占66.02%);表面纹理主要是颗粒状(61例,占59.22%)。结论:Hp阴性胃HG-IEN患者病灶主要位于胃窦,呈隆起型,长径多集中在0.5~0.9 cm和1.0~1.9 cm。病灶表面主要呈现糜烂和结节,颜色大多正常或发白。颗粒状是表面纹理最常见的特征。
Objective:To investigate the endoscopic and histologic features of Helicobacter pylori(Hp)-negative gastric high-grade intraepithelial neoplasia(HG-IEN).Methods:The clinical data were retrospectively collected from 103 patients with Hp-negative gastric HG-IEN who were confirmed by pathology after surgery or endoscopic submucosal dissection resection in No.215 Hospital of Shaanxi Nuclear Industry from March 2020 to April 2023.The endoscopic manifestations and histologic features of the patients were counted and observed.Results:Among the 103 patients,the lesions mainly located on gastric sinus(65 cases,63.11%),and the morphology of the lesions was mainly bulging(69 cases,66.99%),the size of most lesions was 0.5 to 0.9 cm(46 cases,44.66%),and 1.0 to 1.9 cm(38 cases,36.89%).The appearances of the lesions generally were surface erosion(35 cases,33.98%)and surface nodules(36 cases,34.95%);the color of the lesions was commonly normal or whitish(68 cases,66.02%);and the surface texture was mainly granular(61 cases,59.22%).Conclusions:The lesions of Hp-negative gastric HG-IEN are mainly located on the gastric antrum,with elevated shape,manifesting 0.5 to 0.9 cm and 1.0 to 1.9 cm in major diameter commonly.The appearance of the lesions are mainly erosions and nodules on the surface,while the color is mostly normal or whitish.As for surface textural features,granularity is the most common.
