摘要
目的探讨慢性鼻-鼻窦炎(CRS)患者经鼻呼出气一氧化氮(nNO)与肺通气功能的相关性。方法选取2020年12月至2023年8月于福建医科大学附属龙岩第一医院耳鼻咽喉科手术的慢性鼻-鼻窦炎不伴鼻息肉(CRSsNP)患者45例(A组)、慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)患者50例(B组),另外选取同期健康对照组40名(C组),分别对3组行nNO及肺通气功能指标用力肺活量(FVC)、肺活量(VC)、最大通气量(MVV)、第一秒用力呼气量(FEV1)、一秒率(FEV1%)、呼气峰值流速(PEF)、最大呼气流速(MEF)的检测。结果C组nNO、肺通气功能指标FVC、VC、MVV、FEV1、FEV1%、PEF、MEF高于A组和B组(P<0.05);A组nNO、肺通气功能指标FVC、VC、MVV、FEV1、FEV1%、PEF、MEF高于B组(P<0.05)。CRS患者nNO与FEV1、FEV1%、PEF、MEF没有相关性(P>0.05),与FVC、VC、MVV呈正相关(r=2.234、2.547、2.121,P<0.05)。结论同健康者相比,CRS患者nNO降低,肺通气功能具有不同程度下降;对于CRS患者,nNO在一定程度上反映了其炎性病变情况,nNO与其肺通气功能具有相关性。
Objective To explore the correlation between nasal nitric oxide(nNO)and pulmonary ventilation function in patients with chronic rhinosinusitis(CRS).Methods A total of 45 patients with chronic rhinosinusitis without nasal polyps(CRSsNP)(Group A),50 patients with chronic rhinosinusitis with nasal polyps(CRSwNP)(Group B)and 40 healthy controls during the same period(Group C)were selected from the Department of Otolaryngology of Longyan First Affiliated Hospital of Fujian Medical University from December 2020 to August 2023.The three groups were treated with nNO and pulmonary ventilation function indicators,including forced vital capacity(FVC),vital capacity(VC),maximal ventilatory volume(MVV),first second forced expiratory volume(FEV1),forced expiratory rate of the 1st second(FEV1%),peak rate of expiration flow(PEF)and the maximum rate of expiratory flow(MEF).Results The values of nNO,FVC,VC,MVV,FEV1,FEV1%,PEF and MEF in Group C were higher than those in Group A and Group B,with statistical significance(P<0.05);The values of nNO,pulmonary ventilation function indicators FVC,VC,MVV,FEV1,FEV1%,PEF,and MEF in Group A were higher than those in Group B,with statistically significant differences(P<0.05).There was no correlation between nNO and FEV1,FEV1%,PEF,MEF in CRS patients(P>0.05),but it was positively correlated with FVC,VC,and MVV(r=2.234,2.547,2.121,P<0.05).Conclusions Compared with healthy individuals,CRS patients have a decrease in nNO and varying degrees of decline in pulmonary ventilation function.To a certain degree,nNO reflects their inflammatory lesions for CRS patients,and there is a correlation between nNO and their pulmonary ventilation function.
