摘要
目的分析DRG/DIP付费改革背景下中医院运行管理的实践。方法将本院2017年1—6月使用项目付费医保支付方式的300份病案设置为对照组,将2023年1—6月使用DIP付费的300份病案设置为观察组。比较两组医疗服务能力指标、医疗服务安全和费用指标、中医药特色指标。结果观察组入组率高于对照组,差异有统计学意义(P<0.05);三、四级手术数量及总权重对比,两组差异无统计学意义(P>0.05)。观察组药占比高于对照组,观察组的例均费用与例均耗材费用占比均低于对照组,差异有统计学意义(P<0.05);低风险组死亡率、例均耗材费用占比,差异无统计学意义(P>0.05)。观察组的中药饮片处方占比、中医项目治疗人次占比、非药物治疗率、中草+中成药占比、中医参与率都比对照组高,组间差异有统计学意义(P<0.05)。结论DRG/DIP付费模式在中医院运行管理中的引入有助于推动中医药的发展和应用,提高中医院的整体服务质量。
Objective To study practical analysis of operation and management DRG/DIP payment in traditional Chinese medicine hospitals.Methods The paper chose 300 medical records in our hospital from January 2017 to June 2017 with project payment as control group,and 300 medical records with DIP payment from January 2023 to June 2023 as observation group.Medical service capability indicators,medical service safety and cost indicators,and traditional Chinese medicine characteristic indicators were compared.Results Enrollment rate,number of third and fourth level surgeries,total weight,and drug proportion in observation group was higher than control group(P<0.05).The mortality rate,average cost per case,proportion of average consumables per case,and proportion of average drug consumables per case in low-risk group of observation group was lower than control group(P<0.05).The proportion of prescriptions of decoction pieces of traditional Chinese medicine,the proportion of people treated with traditional Chinese medicine,non drug treatment rate,the proportion of Chinese herbal medicine+traditional Chinese patent medicines and simple preparations,participation rate of traditional Chinese medicine in observation group was higher than control group(P<0.05).Conclusion Introduction of DRG/DIP payment model in the operation and management of traditional Chinese medicine hospitals can promote development and application of traditional Chinese medicine,and improve overall service quality of traditional Chinese medicine hospitals.
