摘要
目的分析中药灌肠治疗细菌性痢疾(BD)的组方用药规律,为该病的临床诊疗和基础研究提供参考依据。方法检索中国知网等数据库中运用中药灌肠治疗BD的文献,运用中医传承辅助平台建立数据库,分析用药规律。结果收纳中药灌肠治疗BD的文献共47篇,含可供录入系统的方剂48首,共涉及中药53味。使用频次最高的3种药物依次为黄连、白头翁、黄柏,频次超过10的药物共有8味,对高频药物进行划分,药物功效以清热燥湿、清热解毒、清热凉血为主。在灌肠治疗BD的中药里,苦寒中药以及归大肠经、胃、脾、肝四经的药物应用最多。常用药物组合共11个,其中黄连-白头翁药对出现频度最高;常用药物规则8则,涉及5味中药。聚类分析得出4个新方组合,分别为黄芩-白芍-赤芍-葛根-木香、白头翁-秦皮-黄柏-大蒜、地榆-葛根-木香-秦皮、黄连-秦皮-黄柏-苦参。结论中药灌肠治疗BD主要以苦寒药物为主,组方思路多为白头翁汤之加减,具有良好的临床疗效和研究前景。
Objective To analyze the prescription rules of traditional Chinese medicine enema in the treatment of bacillary dysentery(BD),and to provide reference for clinical diagnosis and treatment and basic research of the disease.Methods The literature on the treatment of BD by traditional Chinese medicine enema was retrieved from CNKI and other databases.The database was established by using the Traditional Chinese Medicine Inheritance Support System platform,and the medication rules were analyzed.Results There were 47 literatures on the treatment of BD by traditional Chinese medicine enema,including 48 prescriptions available for entry into the system,with 53 flavors of traditional Chinese medicine.The three drugs with the highest frequency of use were Coptis chinensis,Pulsatilla chinensis and Phellodendron amurense.There were 8 drugs with a frequency of more than 10.The high-frequency drugs showed that the efficacy of the drugs was mainly based on clearing heat and drying dampness,clearing heat and detoxifying,clearing heat and cooling blood.Among the traditional Chinese medicines for enema treatment of BD,bitter and cold Chinese medicines and medicines belonging to the large intestine,stomach,spleen and liver were the most widely used.There were 11 commonly used drug combinations,among which Coptis chinensis-Pulsatilla chinensis drug pairs had the highest frequency of occurrence;there are 8 commonly used drug rules,involving 5 traditional Chinese medicines.Four new combinations were obtained by cluster analysis,which were Huangqin-Baishao-Chishao-Gegen-Muxiang,Baitouweng-Qinpi-Huangbo-Dasuan,Diyu-Gengen-Muxiang-Qinpi,Huanglian-Qinpi-Huangbo-Kushen.Conclusion Traditional Chinese medicine enema in the treatment of BD is mainly based on bitter and cold drugs.The prescription idea is mostly the modified Baitouweng decoction,which has good clinical efficacy and research prospects.
