摘要
目的探讨“形神共调”针刺法对老年脑卒中后下肢痉挛性瘫痪的临床疗效。方法选取2020年5月—2022年5月许昌市人民医院收治的脑卒中后下肢痉挛性瘫痪患者共76例,以治疗方法不同将其分为对照组(36例)和观察组(40例),对照组采用常规治疗+传统阳明经穴针刺法,观察组采用常规治疗+“形神共调”针刺法。治疗4周后,观察两组临床疗效,对比两组扩展Barthel指数(EBI指数)、痉挛指数(CSI指数)以及肌电图。结果经过4周治疗,观察组治疗总有效率高于对照组(P<0.05);两组EBI指数均较治疗前降低,观察组低于对照组(P<0.05);两组CSI指数均较治疗前降低,观察组CSI低于对照组(P<0.05);肌电图结果显示,两组胫骨前肌、腓肠肌、股四头肌的肌电值(iEMG)和均方根(RMS)均较治疗前增加,观察组均高于对照组(P<0.05)。结论“形神共调”针刺法治疗老年脑卒中后下肢痉挛性瘫痪的临床疗效显著,能提高患者的下肢肌力,改善患者的生活水平。
Objective To discuss the clinical efficacy of the“co-regulation of body and spirit”acupuncture method in the treatment of post-stroke spastic paralysis of the lower extremities in the elderly.Methods Seventy-six elderly patients with post-stroke spastic paralysis of the lower extremities in Xuchang People's Hospital from May 2020 to May 2022 were enrolled,and randomly assigned into two groups according to different treatment approaches.The control group(36 cases)received conventional therapy,while the observation group received conventional therapy combined with acupuncture.After 4 weeks of treatment,the clinical efficacy,the extended Barthel index(EBI),clinical spasm index(CSI)and electromyography of the two groups were compared.Results After 4 weeks of treatment,the overall efficacy rate of the observation group was higher than that of the control group(P<0.05).A reduction in EBI was observed in both groups after treatment,and the post-treatment EBI was lower in the observation group than that in the control group(P<0.05).CSI showed a decrease in both groups after treatment,and the decrease was more remarkable in the observation group than that in the control group(P<0.05).The integrated electromyography(IEMG)and root mean square(RMS)of tibialis anterior muscle,gastrocnemius muscle and quadriceps femoris were increased in both groups after treatment,and the above values of the observation group were higher than those of the control group(P<0.05).Conclusion Application of“co-regulation of body and spirit”acupuncture method in the treatment of post-stroke spastic paralysis of the lower extremities in the elderly can effectively improve the lower extremity muscle strength and quality of life of elderly patients.
