摘要
目的 探讨女童特发性中枢性性早熟(idiopathic central precocious puberty, ICPP)骨密度(bone mineral density, BMD)变化及骨密度Z值对女童ICPP的临床参考价值。方法 收集2021年1月至2023年5月我院确诊ICPP的女童共30例,为ICPP组,分别选取与患儿年龄相同和骨龄相同的正常儿童各30名作为对照组(同年龄对照组、同骨龄对照组),采用独立样本t检验分别对比ICPP组与同年龄对照组、ICPP组与同骨龄对照组间腰椎及股骨近端BMD的差异性,并以同年龄对照组为对照,分析Z值诊断女童ICPP的价值。结果 ICPP组女童腰椎及股骨近端BMD均大于同年龄对照组(t=4.50、3.66,P<0.05),而与同骨龄对照组差异无统计学意义(t=0.04、0.50,P>0.05)。ROC曲线显示,Z值对诊断女童ICPP具有较高的参考价值,当诊断界值为0.55时,其AUC为0.808,灵敏度为0.70,特异度为0.80。结论 ICPP女童BMD已增至同骨龄正常儿童年龄水平,骨密度可作为临床评估女童ICPP及其骨骼发育程度的参考指标。
Objective To investigate the changes and diagnostic value of bone mineral density(BMD)in girls with idiopathic central precocious puberty(ICPP).Methods From January 2021 to May 2023,30 girls with ICPP diagnosed in our hospital were enrolled(ICPP group).Thirty healthy girls as the same age(same age control group)and thirty healthy girls with the same bone age(same bone age control group)were also enrolled.The difference of the lumbar spine and proximal femur BMD between the ICPP group and the two normal controls were compared.The diagnostic value of the Z value in girls with ICPP was analyzed.Results BMD of the lumbar vertebrae and proximal femur in ICPP group were higher than that in the same age control group(t=4.50,3.66,P<0.05),but there were no significant difference between ICPP group and the same bone age control group(t=0.04,0.50,P>0.05).The ROC curve showed that the Z value had the diagnostic efficiency in girls with ICPP.When the diagnostic boundary value was 0.55,its AUC was 0.808,the sensitivity was 0.70,and the specificity was 0.80,respectively.Conclusion BMD of girls with ICPP has increased to the age level of normal children with the same bone age.The bone mineral density Z value can be used as a reference index for the clinical diagnosis of girls with ICPP.
