摘要
目的探讨以旋髂浅动脉及其各主要分支为血管蒂的游离皮瓣修复四肢创面的临床应用。方法对2018年12月-2022年6月收治的25例四肢创面采用以旋髂浅动脉为血管蒂的游离皮瓣修复,并分析病例的临床解剖和应用。结果24例皮瓣成活,1例术后1 d发生动脉危象,探查后皮瓣坏死。9例因皮瓣外形臃肿,于术后3个月进行皮瓣整形。术中旋髂浅动脉的解剖结果:22例发源于股动脉,1例源于旋髂深动脉,2例源于股深动脉。25例均存在深、浅支,其中2例旋髂浅动脉的浅支和旋髂浅动脉的主干分别从源动脉发出(深、浅支不共干),其余深、浅支均共干。结论旋髂浅动脉主要分支为浅支、深支、降支,浅、深支恒定出现,降支存在少数缺如。三支分支均可做为皮瓣的血管蒂,但各具有其解剖及应用特点,在临床工作中,需根据具体情况选择应用。
Objective To analyze the clinical experience of free flap pedicled with superficial circumflex iliac artery and its main branches.Methods From December 2018 to June 2022,the clinical anatomy and application of free flap pedicled with Superficial circumflex iliac artery were analyzed in our hospital.Results The 24 cases of flap survived,1 case had arterial crisis 1 day after operation and the flap was necrotic.9 patients underwent flap reconstruction 3 months later due to bloated appearance.Anatomical results of superficial circumflex iliac artery during operation:22 case originated from the femoral artery,1 case from the deep circumflex iliac artery,and 2 cases from the deep femoral artery.Deep and superficial branches were present in all the 25 cases.In 2 cases,the superficial branches and the main branches of the superficial circumflex iliac artery originated from the source artery(the deep and the superficial branches did not co-stem),and the other deep and superficial branches were co-stem.Conclusion The main branches of the superficial circumflex iliac artery are superficial branch,deep branch and descending branch.The superficial branch and deep branch always appear,and a few descending branches are absent.The three branches can be used as the vascular pedicle of the flap,but each has its own anatomical and application characteristics.In clinical work,the application should be selected according to the specific situation.
