摘要
目的:从质量保证的角度探讨如何合法、合规使用冰冻血小板,最大限度保障受血者的生命权和健康权。方法:统计2019–2021年江西各地区医院血小板制品的应用数据以及南昌地区2018–2021年单采血小板、浓缩血小板质量监控数据,并对南昌地区冰冻血小板的血小板计数等指标进行分析。结果:目前江西各地区医院在用的血小板血液制品共计8种,包括单采血小板、滤白单采血小板、浓缩血小板、汇集滤白浓缩血小板、冰冻单采血小板、冰冻单采去白血小板、冰冻浓缩血小板、冰冻浓缩混合(滤白)血小板,其中冰冻血小板包括冰冻单采血小板、冰冻单采去白血小板、冰冻浓缩血小板和冰冻浓缩混合(滤白)血小板。分析结果显示冰冻后的单采血小板在保存期末血小板计数会下降约27.3%,冰冻后的浓缩血小板在保存期末血小板计数会下降约42.7%。结论:血站和医院应合法、合规使用血小板血液制品,尽量避免冰冻血小板的使用。应急情况下,必须明确应用冰冻血小板的质量保证机制,包括冰冻血小板的制备标准、质量要求、临床适应证和使用前知情同意原则等。全省应建立区域间的血小板应急保障机制,统一应急协调。
Objective To discuss how to use frozen platelets legally and legally from the perspective of quality assurance,and to maximize the right of life and health of the blood recipients.Methods The application data of platelet products in hospitals in Jiangxi Province from 2019 to 2021 and the quality monitoring data of monosampling and concentrated platelets in Nanchang region from 2018 to 2021 were analyzed,and the platelet count and otheRindicators of frozen platelets in Nanchang region were analyzed.Results Platelet blood products in Jiangxi regional hospitals in a total of eight,including apheresis platelet,filteRapheresis platelet,concentrated platelet,pooling filteRwhite concentrated platelet,frozen apheresis platelet,frozen apheresis,frozen platelet,frozen concentrated mixed(filter)platelets,frozen platelets including frozen apheresis platelet,frozen apheresis to white platelet,frozen concentrate and frozen concentrated mixed(filter)platelets.The analysis results showed that the platelet count decreased by about 27.3%at the end of preservation,and the concentrated platelet count decreased by about 42.7%at the end of preservation.Conclusion Blood stations and hospitals should use platelet blood products legally and legally,and avoid the use of frozen platelets.In emergency cases,the quality assurance mechanism forthe application of frozen platelets must be clarified,including the preparation standards,quality requirements,clinical indications and informed consent principles before use.The province should establish an inter-regional platelet emergency support mechanism and unified emergency coordination.
