摘要
目的观察理筋正骨手法联合双氯芬酸二乙胺乳胶剂治疗肱骨外上髁炎的临床疗效。方法将60例肱骨外上髁炎患者采用随机数字表法分为对照组和治疗组,各30例。由于在治疗过程中2组各有1例患者脱离研究,最终各纳入29例。对照组给予体外冲击波联合双氯芬酸二乙胺乳胶剂涂抹治疗,治疗组采用理筋正骨手法联合双氯芬酸二乙胺乳胶剂涂抹治疗。比较2组的临床疗效及治疗前后视觉模拟评分法(VAS)评分、压痛阈值、改良Mayo肘关节功能评分(MEPS)、桡侧腕伸肌群中位频率(MF)及积分肌电(iEMG)值。结果治疗组总有效率为96.55%(28/29),对照组为72.41%(21/29),2组比较差异有统计学意义(P<0.05);治疗后2组VAS评分均明显降低,肘关节疼痛、运动功能、稳定性、日常生活活动能力(ADL)评分及MEPS总分,压痛阈值,桡侧腕伸肌群iEMG及MF值均明显升高,与同组治疗前比较差异有统计学意义(P<0.05或P<0.01),且治疗组各项指标改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.05或P<0.01)。结论理筋正骨手法联合双氯芬酸二乙胺乳胶剂临床疗效显著,可有效缓解肱骨外上髁炎患者疼痛及肌肉疲劳度,降低肘关节周围肌群张力,改善肘关节功能,值得临床推广应用。
Objective To observe the clinical effect of tendon-regulating and bone-setting techniques combined with diclofenac diethylamine emulgel in the treatment of lateral epicondylitis.Methods 60 patients with lateral epicondylitis were divided into control group and treatment group by random number table method,with 30 cases in each group.Since 1 patient in each group left the study during treatment,29 patients were eventually included in each group.The control group was treated with extracorporeal shock wave combined with diclofenac diethylamine emulsion,and the treatment group was treated with tendon-regulating and bone-setting techniques combined with diclofenac diethylamine emulgel.The clinical efficacy,visual analogue scale(VAS)score,tenderness threshold,modified Mayo elbow performance score(MEPS),median frequency(MF)and integrated electromyogram(iEMG)values of the radial extensor carpal muscle were compared between the two groups.Results The total effective rate was 96.55%(28/29)in the treatment group and 72.41%(21/29)in the control group,the difference between the two groups was statistically significant(P<0.05).After treatment,VAS scores in both groups were significantly decreased,e lbow pain,motor function,stability,ADL score and total score,tenderness threshold,iEMG and MF values of radial extensor carpal muscle were significantly increased,and the differences were statistically significant compared with those before treatment(P<0.05 or P<0.01),and all indexes in the treatment group were more significantly increased.Compared with the control group after treatment,the difference was statistically significant(P<0.05 or P<0.01).Conclusion Tendon-regulating and bone-setting techniques combined with diclofenac diethylamine emulgel has significant clinical effect,which can effectively relieve the pain and muscle fatigue of patients with lateral epicondylitis of the humerus,reduce the tension of muscles around the elbow joint,and improve the elbow joint function,which is worthy of clinical application.
