摘要
目的:探究子宫内膜息肉在宫腔镜下行电切切除术、冷刀切除术的治疗价值与安全性。方法:选择2020年1月~2022年4月子宫内膜息肉患者62例,采用数表法分组,对照组、观察组各31例,两组用不同的治疗方法,对照组行宫腔镜电切术、观察组行宫腔镜冷刀切除术,对比疗效、并发症、预后及复发率。结果:观察组息肉冷刀切除手术用时短、术中、术后出血量少,住院时间短,对子宫内膜厚度的影响小,P<0.05;观察组并发症发生率低,P<0.05;观察组术后PBAC评分下降,VEGF水平上升,与对照组组间存在较大差异,P<0.05;调查两组术后半年、1年息肉复发率,观察组复发率低、预后好,术后1年两项百分比纳入统计学,存在统计学意义,P<0.05。结论:宫腔镜冷刀切除术是临床近些年推崇新型微创治疗方式,其效果、安全性优于电切术,对患者子宫内膜损伤小,术后并发症率也相对较低。
Objective:To explore the therapeutic value and safety of hysteroscopic resection and cold knife resection for endometrial polyps.Methods:62 patients with endometrial polyps from January 2020 to April 2022 were selected and divided into a control group and an observation group using the number table method.There were 31 patients in each group,and the two groups received different treatment methods.The control group underwent hysteroscopic resection and the observation group underwent hysteroscopic cold knife resection.The efficacy,complications,prognosis,and recurrence rate were compared.Results:The observation group underwent polypectomy with a cold knife,which resulted in shorter operation time,less intraoperative and postoperative bleeding,shorter hospital stay,and less impact on endometrial thickness(P<0.05);The incidence of complications in the observation group was low,P<0.05;The postoperative PBAC score of the observation group decreased and the VEGF level increased,with a significant difference compared to the control group(P<0.05);Two groups were investigated for polyp recurrence rates six months and one year after surgery,while the observation group had a low recurrence rate and a good prognosis.The percentage of two items included in the statistics for one year after surgery was P<0.05(with statistical significance).Conclusion:Hysteroscopic cold knife resection is a new minimally invasive treatment method that has been highly recommended in clinical practice in recent years.Its effectiveness and safety are better than electrocautery,with minimal damage to the patient’s endometrium and a high postoperative complication rate.
