摘要
目的:探讨彩色多普勒超声多探头联合应用在急性附件扭转的价值。方法:选择2018年1月—2024年1月北京和睦家医院收治的32例疑似急性附件扭转患者为研究对象,均进行经腹凸阵超声探头、经阴道凸阵超声探头及经阴道高频线阵探头彩色多普勒超声检查,以术中表现及病理检查结果为金标准,分析彩色多普勒超声多探头联合诊断急性附件扭转的临床价值。结果:手术及病理证实,32例疑似急性附件扭转患者中确诊为附件扭转21例,彩色多普勒超声多探头联合对附件扭转的检出率为90.48%,高于经腹凸阵探头的检出率为66.67%、经阴道凸阵探头的检出率80.95%及经阴道高频线阵探头的检出率53.13%。ROC曲线分析结果显示,彩色多普勒超声经腹凸阵探头、经阴道凸阵探头、经阴道线阵高频探头联合诊断急性附件扭转的灵敏度为90.5%、特异度为91.0%。O-RADS彩色多普勒超声血流评分越低术中附件活性越低。结论:彩色多普勒超声多探头联合诊断急性附件扭转的价值较高,为临床提供更有意义的诊断依据。
Objective To explore the characteristics and clinical value of multiple color Doppler ultrasound probes combined in acute abdominal adnexal torsion.Methods From January 2018 to January 2024,patients with torsion of adnexal masses in our hospital were selected and examined by convex array transabdominal ultrasound probe,convex array transvaginal ultrasound probe and linear array probe.Intraoperative manifestations and pathological findings were used as the gold standard to compare the accuracy rate of three separate examinations combined with multiple color Doppler ultrasound probes for suspected torsion of adnexal masses.Results The combined application of multi-probe Doppler ultrasound indicated 32 cases of adnexal torsion,and 21 cases of adnexal mass torsion were confirmed after operation,including 12 cases with ovarian and fallopian tube torsion.The accuracy of the diagnosis of adnexal torsion by multi-probe color Doppler ultrasound was 90.48%.The results were significantly higher than that of single application of convex array transabdominal ultrasound probes(66.67%),convex array cavity probe(80.95%)and linear array probe(53.13%).The difference was statistically significant.Conclusion The combined application of multi-probe color Doppler ultrasound can significantly improve the diagnostic accuracy of adnexal torsion in acute abdomen,and provide a more meaningful diagnostic basis for clinic.
