摘要
目的:分析2022年邵阳市中心医院门诊不同剂型中药饮片处方应用情况。方法:选取2022年1—12月邵阳市中心医院门诊开具的1800张中药饮片处方作为研究对象,其中中药传统饮片、中药配方颗粒、中药破壁饮片处方各600张。分析三种剂型中药饮片处方平均药味数、平均剂数、平均单剂价格、不合理类型分布、有毒中药超剂量使用情况。结果:中药传统饮片平均药味数最多,中药破壁饮片平均剂数最多,中药配方颗粒平均单剂价格最高。1800张处方中,不合理处方149张(8.28%),其中中药破壁饮片最多。不合理类型包括临床诊断缺项或不全、超剂量使用、无特殊情况门诊处方>7 d用量或急诊处方>3 d用量、用药与辨证不符、配伍禁忌、用法用量不适宜。超剂量使用的有毒中药饮片处方31张,其中以中药传统饮片处方最多,有毒中药超剂量使用以附片、法半夏较多见。结论:2022年邵阳市中心医院门诊三种剂型中药饮片处方存在临床诊断缺项或不全、超剂量使用等问题,仍需加强临床管理,制定规范、科学合理的中药处方点评制度。
Objective:To analyze the application of different dosage forms of TCM decoction pieces in the outpatient department of Shaoyang Central Hospital in 2022.Methods:From January 2022 to December 2022,one thousand and eight hundred TCM decoction piece prescriptions issued by the outpatient department of Shaoyang Central Hospital were selected as the research object,among which six hundred were traditional TCM decoction pieces,TCM formula granules and TCM wall breaking decoction pieces.The average number of flavor,the average number of doses,the average price of a single dose,the unreasonable distribution of types and the overdosage of toxic Chinese medicine were analyzed.Results:The average number of traditional Chinese medicine pieces was the highest,the average number of broken Chinese medicine pieces was the highest,and the average single dose price of Chinese medicine formula particles was the highest;among the 1800 prescriptions,149(8.28%)were unreasonable,and the most of them were traditional Chinese medicine;Unreasonable types included missing or incomplete clinical diagnosis,over-dose use,outpatient prescription without special conditions>7 d dosage or emergency prescription>3 d dosage,inconsistent use and syndrome differentiation,incompatibility,inappropriate usage and dosage;there were 31 over-dosage prescriptions of toxic Chinese medicine decoction pieces,among which the traditional Chinese medicine decoction pieces were the most common prescriptions,and the over-dosage of toxic Chinese medicine was attached tablets and pinellia.Conclusion:In 2022,Shaoyang Central Hospital outpatient clinic of three dosage forms of Chinese medicine decoction pieces prescription problems such as lack of clinical diagnosis or incomplete,overdosage use,still need to strengthen clinical management,develop a standard,scientific and reasonable TCM prescription review system.
