摘要
目的分析神经根型颈椎病(Cervical Radiculopathy,CR)患者联合使用小针刀、关节错缝术的效果。方法方便选取2021年7月-2022年7月贵阳市花溪区中医院收治的82例CR患者为研究对象,根据随机数表法分为A组(41例,小针刀)和B组(41例,小针刀+关节错缝术),对比两组治疗有效率、治疗前后中医证候积分、治疗前后生活质量量表(Short Form 36-item Health Survey,SF-36)评分。结果治疗前,两组中医证候积分、SF-36评分对比,差异无统计学意义(P均>0.05);B组治疗总有效率为95.12%,高于A组的75.61%,差异有统计学意义(χ^(2)=6.247,P=0.012);治疗后,B组中医证候积分[颈痛(0.72±0.28)分、肩痛(0.75±0.31)分、上肢疼痛麻木(0.78±0.29)分、颈椎活动度(0.73±0.25)分、棘突压痛(0.76±0.23)分]低于A组的[颈痛(1.26±0.35)分、肩痛(1.27±0.39)分、上肢疼痛麻木(1.25±0.36)分、颈椎活动度(1.24±0.32)分、棘突压痛(1.22±0.34)分],差异有统计学意义(P均<0.05);B组治疗后SF-36评分高于A组,差异有统计学意义(P<0.05)。结论CR患者在使用小针刀治疗的基础上,联合关节错缝术,能更好地改善临床症状,使生活质量显著提高,疗效显著。
Objective To analyze the effect of combined use of small needle knife and arthrodesis in patients with neu-rogenic cervical radiculopathy(CR).Methods A total of 82 patients with CR admitted to Huaxi District Hospital of Traditional Chinese Medicine of Guiyang City from July 2021 to July 2022 were conveniently selected as the study objects,and thy were divided into group A(41 cases with small needle knife)and group B(41 cases with small needle knife+arthrosis)according to random number table method.The effective rate of treatment,TCM syndrome score before and after treatment,and score of Short Form 36-item Health Survey(SF-36)before and after treatment were com-pared between the two groups.Results Before treatment,there was no statistically significant difference between the TCM evidence points and SF-36 scores of the two groups(both P>0.05).The total treatment efficiency of group B was 95.12%,higher than that of group A,which was 75.61%,and the difference was statistically significant(χ^(2)=6.247,P=0.012).After treatment,the TCM evidence points of group B[neck pain(0.72±0.28)points,shoulder pain(0.75±0.31)points,upper limb pain and numbness(0.78±0.29)points,cervical spine mobility(0.73±0.25)points,and sphenoidal pressure(0.76±0.23)]points,which were lower than those of group A[neck pain(1.26±0.35)points shoul-der pain(1.27±0.39)points,upper limb pain and numbness(1.25±0.36)points,cervical spine mobility(1.24±0.32)points,and spinous pressure pain(1.22±0.34)points],and the differences were statistically significant(all P<0.05).Af-ter treatment,the SF-36 score of group B was higher than that of group A after treatment,and the difference was statis-tically significant(P<0.05).Conclusion CR patients can better improve the clinical symptoms and make the quality of life significantly improved by combining arthrodesis on the basis of small needle knife treatment,and the therapeutic effect is remarkable.
