摘要
目的:探讨急诊急性呼吸窘迫综合征(ARDS)机械通气患者呼末二氧化碳分压(PETCO_(2))监护仪监测异常原因并分析对策。方法:选取2020年9月~2022年9月本院收治的80例急诊ARDS机械通气患者,所有入院患者均接受PETCO_(2)监测,记录所有患者PETCO_(2)监测出现异常的情况并录入具体原因,对其异常原因进行分析。结果:本研究共80例急诊ARDS机械通气患者共监测1254次,记录中有248次PETCO_(2)异常情况,PETCO_(2)发生率为19.78%。其中气管导管位置不当发生率为9.27%,通气不足发生率为10.48%,呼吸环路漏气发生率为28.63%,分泌物阻塞发生率为14.11%,肺灌溉发生率为6.45%,气囊漏气发生率为14.11%,通气过度发生率为12.50%,机械故障发生率为4.44%。结论:通过PETCO_(2)监测对患者进行持续性的检测,可以通过PETCO_(2)异常显示结合患者的具体情况和其他检测数据进行联合分析,及时有效地发现患者通气、循环或是代谢方面出现的不良状态,从而及时采取有效措施,提升患者救治率,改善其预后。
Objective:To explore the causes of abnormal monitoring of end-expiratory carbon dioxide partial pressure(PETCO_(2))in emergency acute respiratory distress syndrome(ARDS)patients undergoing mechanical ventilation and analyze countermeasures.Methods:80 emergency ARDS mechanical ventilation patients admitted to our hospital from September 2020 to September 2022 were selected.All admitted patients underwent PETCO_(2)monitoring,and the abnormal situation of PETCO_(2)monitoring in all patients was recorded and the specific reasons were recorded.The reasons for the abnormal situation were analyzed.Results:A total of 80 emergency ARDS patients undergoing mechanical ventilation were monitored 1254 times in this study,with 248 abnormal occurrences of PETCO_(2)recorded.The incidence of PETCO_(2)was 19.78%.The incidence of improper placement of tracheal catheters was 9.27%,the incidence of insufficient ventilation was 10.48%,the incidence of respiratory circuit leakage was 28.63%,the incidence of secretion obstruction was 14.11%,the incidence of lung irrigation was 6.45%,the incidence of airbag leakage was 14.11%,the incidence of over ventilation was 12.50%,and the incidence of mechanical failure was 4.44%.Conclusion:Continuous detection of patients through PETCO_(2)monitoring can be carried out by combining PETCO_(2)abnormal display with specific patient conditions and other test data for joint analysis,timely and effective detection of adverse conditions in ventilation,circulation,or metabolism of patients,thus taking timely and effective measures to improve patient treatment rate and prognosis.
