摘要
目的观察滋肾清胃解毒方治疗侵袭性牙周炎(AgP)的疗效并探讨其作用机制。方法92例患者以随机数字表法分为对照组与治疗组各46例。对照组给予西医常规治疗(基础治疗+甲硝唑),治疗组在对照组基础上辅以滋肾清胃解毒方。两组均治疗2周。比较两组治疗前后中医证候评分、AgP检测指标、外周血中性粒细胞胞外诱捕网(NETs)标志物cf-DNA、MPO-DNA、NE及外周血白细胞介素-6(IL-6)、白细胞介素-8(IL8)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)的变化。分析NETs标志物与AgP检测指标相关性,并观察临床疗效。结果治疗组总有效率为91.30%,高于对照组的76.09%(P<0.05)。两组治疗前中医证候评分,AgP检测指标,外周血NETs标志物cf-DNA、MPO-DNA、NE,炎症因子IL-6、IL-8、IL-1β、TNF-α水平比较,差别均不大(均P>0.05)。两组治疗后中医证候评分,AgP检测指标,外周血NETs标志物cf-DNA、MPO-DNA、NE,炎症因子IL-6、IL-8、IL-1β、TNF-α水平与治疗前比较均降低(P<0.05),且治疗组上述指标除IL-6、IL-8水平外均优于对照组(均P<0.05);NETs标志物与AgP检测指标之间呈正相关。结论滋肾清胃解毒方联合常规西医治疗AgP(肾阴虚胃火盛证)疗效确切,可改善患者的牙周状况,减轻机体炎症反应。
Objective:To observe the clinical efficacy of Zishen Qingwei Jiedu Decoction on aggressive periodon⁃titis(AgP).Methods:92 patients were randomly divided into the control group and the treatment group with 46 cases in each group.The control group received western medicine treatment(basic treatment+metronidazole),and the treatment group was added with Zishen Qingwei Jiedu Decoction on this basis.All were treated for 2 weeks.The TCM syndrome score,AgP detection indicators,peripheral blood NETs markers(cf-DNA,MPO-DNA,NE),peripheral blood IL-6,IL-8,IL-1β,TNF-αbefore and after treatment were compared between the two groups.The correlation between NETs markers and AgP detection indicators was analyzed,and the clinical effica⁃cy of the two groups was observed.Results:The total effective rate of treatment group was higher than that of con⁃trol group(P<0.05).After treatment,the TCM symptom score,AgP detection index,NETs markers cf-DNA,MPODNA and NE and inflammatory factor levels(IL-6,IL-8,IL-1β,TNF-α)in the two groups were significantly de⁃creased compared with before treatment(P<0.05),and the decrease was more obvious in treatment group than con⁃trol group(P<0.05).There was a positive correlation between the NETs markers and the detection index of AgP.Conclusion:Zishen Qingwei Jiedu Decoction combined with western medicine in the treatment of AgP(syndrome of kidney yin deficiency and stomach fire excess)has definite effect,which can improve the periodontal condition of patients and reduce the inflammatory reaction of the body.
