俯卧位引流联合纤维支气管镜在神经重症出血性卒中相关性肺炎患者中的应用

摘要 目的探究俯卧位引流联合纤维支气管镜对卒中相关性肺炎患者肺部感染控制及呼吸血气相关指标效果,以期达到早期控制感染的目的。方法选取2022年10月—2023年4月在郑州大学第一附属医院神经重症监护室确诊为出血性卒中相关性肺炎的60例患者,试验组给予俯卧位引流联合床旁纤维支气管镜,对照组除不给予俯卧位引流外,两组其他治疗方案相同。比较两组患者治疗前后动脉血氧分压、血氧饱和度、白细胞介素-6、C反应蛋白的指标变化,比较两组患者住院时长及机械通气时间。结果两组患者治疗前动脉血氧分压、血氧饱和度、白细胞介素-6及C反应蛋白比较差异无统计学意义(P>0.05);两组患者治疗后的动脉血氧分压、血氧饱和度、白细胞介素-6及C反应蛋白比较差异有统计学意义(P<0.05),两组患者住院时长及机械通气时间比较差异有统计学意义(P<0.05)。结论采用俯卧位引流联合纤维支气管镜在出血性卒中相关性肺炎患者中效果显著,可以降低患者的炎症相关指标,提高其动脉血氧分压及血氧饱和度,降低患者的住院时长及机械通气时间,减少医疗费用,降低医保支出,减轻患者负担。 Objective To explore the effect of prone drainage combined with fiberbronchoscopy on pulmonary infection control and respiratory blood gas related indexes in patients with stroke-related pneumonia,so as to achieve the purpose of early infection control.Methods A total of 60 patients diagnosed with hemorrhagic stroke-associated pneumonia in the Neurological intensive care Unit of the First Affiliated Hospital of Zhengzhou University from October 2022 to April 2023 were selected.The experimental group was given prone drainage combined with bedside fiberoptic bronchoscopy,while the control group was given no prone drainage.Other treatment options were the same between the two groups.The changes of arterial blood oxygen partial pressure,blood oxygen saturation,interleukin-6 and C-reactive protein before and after treatment were compared between the two groups,and the length of hospital stay and mechanical ventilation time were compared between the two groups.Results There were no significant differences in arterial oxygen partial pressure,oxygen saturation,interleukin-6 and C-reactive protein between 2 groups before treatment(P>0.05).There were statistically significant differences in arterial blood oxygen partial pressure,blood oxygen saturation,interleu⁃kin-6 and C-reactive protein between the two groups after treatment(P<0.05),and there were statistically signifi⁃cant differences in hospital stay and mechanical ventilation time between the two groups(P<0.05).Conclusion Prone drainage combined with fiberbronchoscopy has significant effects in patients with hemorrhagic stroke-associated pneumonia,which can reduce inflammation-related indicators,increase arterial blood oxygen partial pressure and blood oxygen saturation,reduce the length of hospital stay and mechanical ventilation time of patients,re⁃duce medical expenses,reduce medical insurance expenditure,and alleviate the burden of patients.
