摘要
目的:分析一期改良Pe(n)a手术治疗新生儿中高位肛门闭锁的效果。方法:选取2018年1月—2022年12月徐州市儿童医院新生儿外科收治的68例中高位肛门闭锁新生儿作为研究对象,随机分为改良组与对照组,各34例。对照组实施后矢状入路肛门成形术,改良组实施一期改良Pe(n)a手术。比较两组疗效、手术指标、近期并发症发生情况及远期肛门控便能力。结果:改良组治疗总有效率高于对照组,手术时间、创口恢复时间及住院时间短于对照组,术中出血量少于对照组,近期并发症发生率,便意、控便能力、大便失禁、污粪、便秘、排便频率、社会问题评分及总分低于对照组,差异有统计学意义(P<0.05)。结论:一期改良Pe(n)a手术治疗新生儿中高位肛门闭锁的效果较好,可减少近期并发症,缩短术后恢复时间,改善患儿远期肛门控便能力。
Objective:To analyze the effect of one-stage modified Pe(n)a surgery in the treatment of middle and high anal atresia in newborns.Methods:From January 2018 to December 2022,68 newborns with middle and high anal atresia admitted to the Neonatal Surgery Department of Xuzhou Children's Hospital were selected as the study subjects.They were randomly divided into a modified group and a control group with 34 cases in each group.The control group received posterior sagittal anorectoplasty(PSARP),while the modified group received one-stage modified Pe(n)a surgery.The efficacy,surgical indicators,recent complications,and long-term anal control ability of the two groups were compared.Results:The total effective rate of treatment in the modified group was higher than that in the control group,the operation time,wound recovery time and hospital stay in the modified group were shorter than those in the control group,the intraoperative blood loss in the modified group was less than that in the control group,and the incidence of recent complications,the awareness of defecation,the ability to control stool,fecal incontinence,dirty stool,constipation,defecation frequency,social problem scores and total scores in the modified group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The effect of onestage modified Pe(n)a surgery in the treatment of middle and high anal atresia in newborns is good.It can reduce the short-term complications,shorten the postoperative recovery time,and improve the long-term anal control ability of the child.
