摘要
目的探讨超声心动图联合D-二聚体测定在急性肺栓塞诊断中的应用价值。方法将2022年1月—2023年7月在本院治疗的38例急性肺栓塞(APE)患者作为研究对象,依据纳入标准排除8例,共有30例符合标准患者纳入研究,将其作为观察组,将同期20例健康体检者作为对照组,均实施超声心动图检查和D-二聚体(D-D)检测,对比两组的超声心动图参数及D-D水平、计算两者联合的诊断效能。结果观察组右室前后径(RV-ap)、右室左右径(RV-b)、肺动脉内径(PA)、肺动脉收缩压(PASP)、D-D均大于对照组,左室舒张末内径(LVEDd)、左室左右径(LV-b)、肺动脉前向血流加速时间(ACT)小于对照组(P<0.05);超声心动图联合D-D诊断APE的准确度、阴性预测值均高于单一诊断(P<0.05),而三组敏感度、特异度、阳性预测值相当(P>0.05)。结论超声心动图联合D-D在APE诊断中的应用价值显著,能有效提高对APE的诊断效能,利于指导临床早期治疗。
Objective To explore application value of echocardiography combined with D-dimer measurement in diagnosis of acute pulmonary embolism.Methods The paper chose 38 patients with acute pulmonary embolism(APE)in our hospital from January 2022 to July 2023 as study subjects,8 cases were excluded according to the inclusion criteria,30 patients met the criteria were included in observation group,and 20 healthy individuals Physical examinations during the same period in control group.Both groups were treated with echocardiography and D-dimer(D-D)testing,echocardiography parameters and D-D levels were compared between two groups,diagnostic efficiency was calculated.Results Right ventricular anteroposterior diameter(RV-ap),right ventricular left-right diameter(RV-b),pulmonary artery diameter(PA),pulmonary artery systolic pressure(PASP),and D-D in observation group was higher than control group,while left ventricular end diastolic diameter(LVEDd),left ventricular left-right diameter(LV-b),and pulmonary artery anterior flow acceleration time(ACT)was lower than control group(P<0.05).Accuracy and negative predictive values of echocardiography combined with D-D in diagnosing APE was higher than single diagnosis(P<0.05),while sensitivity,specificity,and positive predictive values of three groups were comparable(P>0.05).Conclusion Echocardiography combined with D-D can achieve significant application value in diagnosis of APE,can improve diagnostic efficiency of APE and facilitate early guidance for clinical treatment effectively.
