血浆游离3-甲氧基肾上腺素和游离3-甲氧基去甲肾上腺素在嗜铬细胞瘤/副神经节瘤临床诊断中的价值研究 被引量:2

Study on the value of plasma free 3-methoxy-epinephrine and free 3-methoxy-norepinephrine in clinical diagnosis of pheochromocytoma/paraganglioma

摘要 目的 探讨血浆游离3-甲氧基肾上腺素(MN)和游离3-甲氧基去甲肾上腺素(NMN)在嗜铬细胞瘤/副神经节瘤(PPGL)临床诊断中的价值。方法 选取65例PPGL患者作为研究组,同期65例高血压非PPGL患者作为对照组。比较两组受试者一般临床资料及血浆游离MN、NMN水平。根据肿瘤不同分化程度将研究组患者分为高分化组(39例)、中分化组(15例)及低分化组(11例),比较3组患者血浆游离MN、NMN水平。相关性分析采用Pearson和Spearman相关分析。采用受试者工作特征(ROC)曲线分析血浆游离MN、NMN水平对PPGL的预测价值。结果 研究组收缩压、舒张压及血浆游离MN、NMN水平均高于对照组;随着PPGL患者肿瘤分化程度降低,血浆游离MN、NMN水平逐渐升高(P<0.05)。Pearson相关分析结果显示,PPGL患者血浆游离MN、NMN水平与收缩压、舒张压均呈正相关;Spearman相关分析结果显示,血浆游离MN、NMN水平与肿瘤分化程度均呈负相关(P<0.05)。ROC曲线分析结果显示,血浆游离MN、NMN水平联合预测PPGL的ROC曲线下面积(AUC)大于二者单独预测的AUC。结论血浆游离MN、NMN水平检测可作为诊断PPGL的重要指标,且与肿瘤分化程度密切相关。 Objective To explore the value of plasma free 3-methoxy-epinephrine(MN)and free 3-methoxy-norepinephrine(NMN)in clinical diagnosis of pheochromocytoma/paraganglioma(PPGL).Methods A total of 65 PPGL patients were selected as study group,and 65 non-PPGL patients with hypertension were selected as control group.General clinical data and plasma free MN and NMN levels were compared between the 2 groups.According to the different degrees of tumor differentiation,the study group patients were divided into highly differentiated group(39 cases),moderately differentiated group(15 cases)and poorly differentiated group(11 cases).The plasma free MN and NMN levels were compared between the 3 groups.Correlation analysis was performed by Pearson and Spearman correlation analysis.The predictive value of plasma free MN and NMN levels to PPGL was analyzed by receiver operating characteristic(ROC)curve.Results The levels of SBP,DBP,plasma free MN and NMN levels in study group were higher than those in control group;With the decrease of tumor differentiation in PPGL patients,plasma free MN and NMN levels gradually increased(P<0.05).Pearson correlation analysis showed that plasma free MN and NMN levels were positively correlated with SBP and DBP in PPGL patients;Spearman correlation analysis showed that plasma free MN and NMN levels were negatively correlated with the degree of tumor differentiation(P<O.05).ROC curve analysis results showed that the area under the ROC curve(AUC)predicted PPCL by the combined prediction of plasma free MN and NMN levels was larger than the AUC predicted by them alone.Conclusion The detection of plasma free MN and NMN levels can be used as an important indicator for the diagnosis of PPGL,and is closely related to the degree of tumor differentiation.
