摘要
目的观察雷火灸定点回旋透热技术治疗膝骨关节炎(KOA)(寒湿痹阻证)的临床效果并探讨其作用机制及安全性。方法100例患者随机分为对照组与试验组各50例。对照组予以雷火灸回旋灸治疗,试验组予以雷火灸定点回旋透热技术治疗,两组均治疗7d。比较两组治疗前后关节疼痛程度的视觉模拟量表(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分及治疗第1.3、7日预热前后、定点治疗7min及治疗结束5min后的温度。结果两组治疗7d后VAS评分及WOMAC指数较治疗前降低,且试验组均低于对照组(均P<0.05)。两组治疗前后的预热前、预热后5min温度值比较,差别均不大(均P>0.05)。两组患者治疗后定点7min及治疗结束5min后温度与预热前比较均明显升高(P<0.05),两组间比较差别均不大(均P>0.05)。结论雷火灸定点回旋透热技术治疗KOA(寒湿痹阻证)可有效缓解患者临床症状,改善其关节功能,且无明显不良反应发生。
Objective:To observe the clinical effect,mechanism of action,and safety of targeted and rotational heat penetration technique with thunder-fire moxibustion in the treatment of knee osteoarthritis(KOA)with colddampness blockage syndrome.Methods:100 patients were randomly divided into a control group and an experi-mental group,with 50 patients in each group.The control group received thunder-fire moxibustion with rotational moxibustion,while the experimental group received targeted and rotational heat penetration technique with thun-derfire moxibustion.Both groups were treated for 7 days.The visual analog scale(VAS)for joint pain,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores,and temperature before and after the 1st,3rd,and 7th day of treatment,as well as the temperature during the 7-minute targeted treatment and 5 min-utes after treatment completion,were compared between the two groups.Results:After 7 days of treatment,the VAS scores and WOMAC index were significantly reduced compared with before treatment,and the experimental group had lower scores than the control group(all P<0.05).There were no significant dfferences in temperature before and after preheating for 5 minutes between the two groups(all P>0.05).The temperature during the 7-minute targeted treatment and 5 minutes after treatment completion significantly increased compared with before preheating in both groups(all P<0.05),with no significant differences between the two groups(all P>0.05).Conclusion:Targeted and rotational heat penetration technique with thunder-fire moxibustion effectively relieves clinical symptoms and improves joint function in patients with KOA and cold-dampness blockage syndrome,with no signif-icant adverse reactions.
