摘要
目的探讨血必净联合氢化可的松对脓毒症患者炎性因子水平的影响。方法选择2022年1月至2023年4月广东医科大学附属第二医院收治的90例脓毒症患者作研究对象,采用随机数字法分成两组,治疗组、对照组各45例。对照组予氢化可的松治疗,治疗组于此基础上予血必净治疗,治疗7 d后对比两组炎性因子水平、血管内皮功能及预后情况。结果治疗后两组白介素-6(IL-6)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)水平均明显降低,且治疗组PCT、IL-6、hs-CRP水平更低,差异有统计学意义(P<0.05);治疗后两组可溶性血栓调节素(sTM)、血管性血友病因子(vWF)及内皮特异性分子(ESM-1)水平均明显降低,且治疗组sTM、vWF、ESM-1水平更低,差异有统计学意义(P<0.05);治疗后两组急性生理与慢性健康状况评分系统Ⅱ(APACHEⅡ)、序贯器官衰竭检测(SOFA)评分均明显降低,且治疗组APACHEⅡ、SOFA评分更低,差异有统计学意义(P<0.05)。结论血必净联合氢化可的松可调控脓毒症患者炎性因子水平,减轻血管内皮损伤,改善预后。
Objective To explore the effect of xuebijing combined with hydrocortisone on the levels of inflammatory factors in patients with sepsis.Methods 90 patients with sepsis admitted to The Second Affiliated Hospital of Guangdong Medical University from January 2022 to April 2023 were selected as the study subjects.They were randomly divided into two groups using a random number method,with 45 patients in the treatment group and 45 patients in the control group.The control group was treated with hydrocortisone,while the treatment group was treated with xuebijing on this basis.After 7 days of treatment,the levels of inflammatory factors,vascular endothelial function,and prognosis of the two groups were compared.Results After treatment,the levels of interleukin-6(IL-6),procalcitonin(PCT)and high-sensitivity C-reactive protein(hs CRP)in the two groups were significantly reduced,and the levels of PCT,IL-6 and hs CRP in the treatment group were lower,with statistical difference(P<0.05);the levels of soluble thrombomodulin(sTM),Von Willebrand factor(vWF)and endothelial specific molecule(ESM-1)in the two groups were significantly lower,and the levels of sTM,vWF and ESM-1 in the treatment group were lower,with statistical difference(P<0.05);the APACHEⅡand SOFA scores of the two groups significantly decreased,and the APACHEⅡand SOFA scores of the treatment group were lower,with a statistical difference(P<0.05).Conclusion Xuebijing combined with hydrocortisone can regulate the levels of inflammatory factors,alleviate vascular endothelial damage,and improve prognosis in patients with sepsis.
