摘要
目的 分析徐州医科大学附属第三医院2022年糖尿病伴感染患者抗菌药物用药合理性,旨在为其抗菌药物的合理使用提供参考。方法 选取2022年1—12月于徐州医科大学附属第三医院住院治疗的糖尿病伴感染患者239例作为研究对象,采用回顾性调查方法,通过HIS收集患者的基本资料,分析抗菌药物使用情况,采用合理用药系统软件评价抗菌药物的用药必要性、有效性及安全性,统计药物相关问题(DRPs)及不合理用药类型。结果 239例患者中,144例患者使用1种抗菌药物,占比60.25%;60例患者使用2种抗菌药物,占比25.10%,35例患者使用3种抗菌药物,占比14.65%。头孢他啶使用频次最高,占比23.61%(144/610),其次为头孢哌酮钠舒巴坦、头孢唑林、左氧氟沙星等。239例患者中,61例患者存在DRPs,发生率为25.52%,共发生DRPs 88次,其中三代头孢发生次数最多,占比51.14%(45/88),其次为β-内酰胺酶抑制剂复合制剂、一代头孢、喹诺酮类等。239例患者中,81例患者存在不合理使用抗菌药物情况,占比33.89%,其中53例患者抗菌药物选择不适宜占比最高,为65.43%,其次为用法用量不适宜、联合用药不适宜、给药途径不适宜、疗程过长。结论 医院2022年糖尿病伴感染患者存在抗菌药物使用不合理现象,应根据患者病情选择适宜的抗菌药物,调整联合用药品种及用药剂量,以保证抗菌药物合理、有效、安全用药。
Objective To analyze the rationality of antibacterial drug use in diabetic patients with infection in the Third Affiliated Hospital of Xuzhou Medical University in 2022,in order to provide reference for the rational use of antibacterial drugs.Methods 239 cases of diabetic patients with infection admitted to the Third Affiliated Hospital of Xuzhou Medical University from January to December 2022 were selected as the study subjects.A retrospective survey method was used to collect patients′basic information through the HIS for analysis of antimicrobial drug usage.The necessity,effectiveness,and safety of antimicrobial drugs were evaluated by using rational drug utilization software,and to assess drug-related problems(DRPs)and types of inappropriate medication use.Results Among the 239 patients,144 patients used one type of antimicrobial drug,accounting for 60.25%;60 patients used two types of antimicrobial drugs,accounting for 25.10%;and 35 patients used three types of antimicrobial drugs,accounting for 14.65%.The most frequently used antimicrobial drug was cefotetan,accounting for 23.61%(144/610),followed by cefoperazone-sulbactam,cefuzonam,levofloxacin,and the others.Among the 239 patients,61 patients had DRPs,with an incidence rate of 25.52%.There were a total of 88 frequency occurrences of DRPs,with third-generation cephalosporins having the highest occurrence rate at 51.14%(45/88),followed byβ-lactamase inhibitor combinations,first-generation cephalosporins,quinolones,and the others.Among the 239 patients,81 patients had inappropriate use of antimicrobial drugs,accounting for 33.89%.Among them,the highest proportion was inappropriate drug selection in 53 patients,with 65.43%,followed by inappropriate dosage or administration,inappropriate combination therapy,inappropriate route of administration,and excessively prolonged treatment courses.Conclusion There are unreasonable practices regarding the use of antimicrobial drugs among diabetic patients with infection in the hospital in 2022.In the future,appropriate antimicrobial drugs should be selected based on patients′conditions,and adjustments should be made to the types and dosages of combination therapies,ensuring a more rational,effective,and safe use of antimicrobial drugs for patient treatment.
