摘要
目的探究尿毒症患者行自体动静脉内瘘成形术前采用臂丛神经阻滞麻醉对手术的影响。方法回顾性分析2021年3月—2023年6月南通市第六人民医院收治的64例尿毒症患者的临床资料。依不同的麻醉方式分为对照组及观察组,每组32例。两组手术方式均为自体动静脉内瘘成形术。对照组予以局部浸润麻醉,观察组予以臂丛神经阻滞麻醉。对比两组麻醉效果、疼痛程度、手术时间、麻醉后动静脉血管内径及术后24 h内瘘血管血流量。结果观察组麻醉总有效率(100.00%)高于对照组(87.50%),差异有统计学意义(χ^(2)=4.267,P<0.05)。观察组手术时间、疼痛程度均优于对照组,差异有统计学意义(P<0.05)。观察组麻醉后动、静脉血管内径及术后24 h内瘘血管血流量均优于对照组,差异有统计学意义(P<0.05)。结论对于行自体动静脉内瘘成形术的尿毒症患者,在术前采取臂丛神经阻滞麻醉有助于提高麻醉效果,且缩短手术时间、减轻疼痛感。
Objective To investigate the effect of brachial plexus nerve block anesthesia on surgery before autologous arteriovenous endovascular fistuloplasty in uremic patients.Methods The clinical data of 64 patients with uremia admitted to Nantong Sixth People's Hospital from March 2021 to June 2023 were retrospectively analyzed.According to different anesthesia methods,they were divided into control group and observation group,32 cases in each group.The surgical methods of both groups were autogenous arteriovenous fistula plasty.The control group was given local infiltration anesthesia,and the observation group was given brachial plexus block anesthesia.The anesthesia effect,pain degree,operation time,internal diameter of arteriovenous vessels after anesthesia,and blood flow of fistula vessels within 24 h after operation were compared between the two groups.Results The total effective rate of anesthesia in the observation group(100.00%)was higher than that in the control group(87.50%),and the difference was statistically significant(χ^(2)=4.267,P<0.05).The operation time and pain degree of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).The observation group had better arterial and venous vessel diameter after anesthesia and fistula blood flow within 24 h after operation compared to the control group,and the differences were statistically significant(P<0.05).Conclusion For uremic patients who underwent autologous arteriovenous endovascular fistuloplasty,brachial plexus nerve block anesthesia before surgery can help to improve the effect of anesthesia,and shorten the operation time and reduce the pain.
