摘要
目的分析老年2型糖尿病患者体脂率与肌力及躯体功能间的关系。方法通过便利抽样法,选取2021年6月—2022年12月在福建医科大学附属第一医院马尾分院接受治疗的98例老年2型糖尿病患者,所有患者均接受四肢骨骼肌肌肉质量、体脂率测定,简易体能状况量表(short physical performance battery,SPPB)调查以及椅子起坐试验。分析实验室指标、体脂率与肌力及躯体功能的相关性,并通过多元线性回归分析法分析肌力及躯体功能的影响因素。结果Pearson相关分析结果显示,老年2型糖尿病患者体脂率与SPPB评分、握力、起坐试验、四肢骨骼肌质量指数、四肢骨骼肌质量呈负相关(r<0,P<0.05),与胰岛素抵抗指数、C反应蛋白呈正相关(r>0,P<0.05)。多元线性回归分析结果显示,年龄、性别、四肢骨骼肌质量、四肢骨骼肌质量指数、体脂率是影响老年2型糖尿病患者握力的主要影响因素(P<0.05);年龄、体脂率是影响患者起坐试验、躯体功能的主要影响因素(P<0.05)。结论老年2型糖尿病患者体脂率与肌力及躯体功能存在显著相关性,是导致患者躯体功能以及肌力降低的重要影响因素,临床上应加强老年2型糖尿病患者体脂管理,以改善肌力及躯体功能。
Objective To analyze the relationship between body fat rate,muscle strength and body function in elderly patients with type 2 diabetes.Methods By convenient sampling method,a total of 98 elderly patients with type 2 diabetes who were treated in Mawei Branch of the First Affiliated Hospital of Fujian Medical University from June 2021 to December 2022 were selected.All patients received measurement of muscle mass and body fat rate of skeletal muscles of limbs,investigation of short physical performance battery(SPPB)and chair sitting up test.The correlation between laboratory indicators,body fat percentage,muscle strength,and physical function was analyzed,and to analyze the influencing factors of muscle strength and physical function through multiple linear regression analysis.Results Pearson correlation analysis showed that the body fat rate of elderly patients with type 2 diabetes was negatively correlated with SPPB score,grip strength,sit up test,limb skeletal muscle mass index and limb skeletal muscle mass(r<0,P<0.05),and positively correlated with insulin resistance index and C-reactive protein(r>0,P<0.05).The results of multiple linear regression analysis showed that age,gender,limb skeletal muscle mass,limb skeletal muscle mass index and body fat rate were the main influencing factors on grip strength in elderly patients with type 2 diabetes(P<0.05),and age and body fat rate were the main influencing factors on sit up test and body function(P<0.05).Conclusion There is a significant correlation between body fat rate and muscle strength and body function in elderly patients with type 2 diabetes,which is an important influencing factor leading to the decline of body function and muscle strength.In clinical practice,body fat management should be strengthened in elderly patients with type 2 diabetes to improve muscle strength and body function.
