摘要
目的:分析产前超声中孕期检查用于筛查胎儿是否畸形及正常发育的价值。方法:选取2021年6月~2022年6月在本院接受产前超声检查的500例中孕期产妇(500例胎儿),统计分析本组产妇的超声检测结果,并与最终引产(分娩)结果进行对比。结果:本组胎儿畸形和发育异常共计16例,胎儿畸形和发育异常率为3.22%(16/500),超声检查显示胎儿畸形和发育异常率为87.5%(14/16),漏诊率为12.5%(2/16);经羊水穿刺、有创穿刺、基因检测等临床检查87例,超声检测79例,超声检出率为90.80%;ROC分析表明,产前超声中孕期检查用于筛查胎儿畸形及异常发育的灵敏度为91.98%、准确度为94.87%、特异度为89.88%、阴性预测值为93.84%、阳性预测值为92.75%,产前超声中孕期检查诊断胎儿出生缺陷AUC为0.887(95%CI:0.171~0.962)。有3例患儿中孕期超声检查存在鼻骨缺如,同时检出胎儿心脏畸形、十二指肠闭锁,该组3例患儿初步考虑有染色体疾病,经羊水穿刺证实为21-三体综合征。结论:产前超声中孕期检查能够较为准确地筛查胎儿是否畸形及正常发育,具有非常重要的临床价值。
Objective:To analyze the value of prenatal ultrasound in screening fetal malformation and normal development.Methods:From June 2021 to June 2022,500 pregnant women(500 fetuses)who received prenatal ultrasound examination in our hospital were selected,and the results of ultrasound examination were statistically analyzed,and compared with the final results of induced labor(delivery).Results:There were 16 cases of fetal malformation and dysplasia in this group.The rate of fetal malformation and dysplasia was 3.22%(16/500).Ultrasonography showed that the rate of fetal malformation and dysplasia was 87.5%(14/16),and the rate of missed diagnosis was 12.5%(2/16);87 cases were examined by amniocentesis,invasive puncture,gene examination and other clinical examinations,79 cases were examined by ultrasound,and the ultrasonic detection rate was 90.80%;ROC analysis showed that the sensitivity,accuracy,specificity,negative predictive value and positive predictive value of prenatal ultrasound for screening fetal malformations and abnormal development were 91.98%,94.87%,89.88%,93.84%and 92.75%respectively.The AUC of prenatal ultrasound for diagnosing fetal birth defects was 0.887(95%CI:0.171~0.962).There were 3 cases of children with nasal bone deficiency during pregnancy,and fetal heart malformation and duodenal atresia were also detected.Three children in this group were initially considered to have chromosome disease,which was confirmed as trisomy 21 syndrome by amniocentesis.Conclusion:Prenatal ultrasound in pregnancy can accurately screen whether the fetus is abnormal and normal development,which has very important clinical value.
