摘要
目的:探讨围术期保温措施对腹腔镜异位妊娠手术患者体核温度及应激反应的影响。方法:选自2020年1月-2022年8月于本院择期腹腔镜异位妊娠手术患者150例,分为观察组及对照组各75例,分别给予围手术期复合保温措施或常规保温措施。分析两组围术期体核温度、心率(HR)、血压、凝血功能指标及血清肾上腺素(AD)和去甲肾上腺素(NE)水平。结果:观察组体核温度麻醉1h(36.17±0.78℃)及手术结束时(36.13±0.82℃)均大于对照组(35.01±0.82℃、34.87±0.84℃),术中HR(71.4±7.5次/min)、收缩压(126.6±10.1 mmHg)及舒张压(72.9±7.9 mmHg)、凝血功能指标、AD(104.62±20.55 pmol/L)、NE(160.77±37.03 pmol/L)及气管拔管时间(27.29±6.90 min)及麻醉苏醒时间(36.52±7.13 min)均低于对照组(79.5±8.9次/min、141.8±19.1 mmHg、84.0±9.4 mmHg、119.43±31.91 pmol/L、179.38±41.22 pmol/L、25.94±6.33 min、33.79±6.84 min)(均P<0.05)。结论:腹腔镜异位妊娠手术围术期积极采取保温措施可预防患者发生低体温,降低应激反应,利于患者术后恢复。
Objective:To investigate the effects of perioperative heat preservation measures during laparoscopic ectopic pregnancy surgery of patients on their core temperature and stress response.Methods:A total of 150patients who wanted elective laparoscopic ectopic pregnancy surgery were selected and were divided into two groups(75cases in each group)from January 2020to August 2022.The patients in the observation group were given perioperative composite heat preservation measures,and the patients in the control group were given perioperative routine heat preservation.169339 TDmeasures.The values of intraoperative core temperature,heart rate,blood pressure,coagulation indexes,and the levels of serum epinephrine and norepinephrine concentrations of the patients in the two groups were analyzed.Results:The core temperature of the patients in the observation group at 1hafter anesthesia(36.13±0.82℃)and at the end of operation(36.17±0.78℃)were significantly higher than those(35.01±0.82℃and 34.87±0.84℃)of the patients in the control group.The values of intraoperative HR(71.4±7.5beats/min),systolic blood pressure(126.6±10.1mm-Hg),and diastolic blood pressure(72.9±7.9mmHg),the levels of AD(104.62±20.55pmol/L)and NE(160.77±37.03)pmol/L),the extubation time(27.29±6.90min),and the anesthesia recovery time(36.52±7.13min)of the patients in observation group were significantly lower than those(79.5±8.9times/min,141.8±19.1mm Hg,84.0±9.4mm Hg,119.43±31.91)pmol/L,179.38±41.22pmol/L,25.94±6.33min,and 33.79±6.84min)of the patients in the control group.The values of the coagulation function indexes of the patients in the observation group were significantly lower than those of the patients in the control group(all P<0.05).Conclusion:Active perioperative heat preservation measures for the patients during laparoscopic ectopic pregnancy surgery can prevent their hypothermia and reduce their stress response,and which is beneficial to their postoperative recovery.
