摘要
目的:探讨腹腔镜无张力疝修补术治疗腹股沟疝患者的效果及对血清疼痛指标和并发症的影响。方法:采用随机抽样方法选取2017年1月—2023年1月南通市肿瘤医院(南院)收治的100例腹股沟疝患者作为研究对象。根据随机数表法将其分为试验组和对照组,各50例。对照组采用开放式无张力疝修补术,试验组采用腹腔镜无张力疝修补术。比较两组围手术期指标,术前、术后疼痛程度,术前和术后1 h血清疼痛指标及并发症。结果:试验组手术时间、住院时间均短于对照组,术中出血量少于对照组,肛门排气时间早于对照组,差异有统计学意义(P<0.05)。术后1 d,试验组视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。术后1 h,试验组去甲肾上腺素(NE)、神经生长因子(NGF)、神经肽Y(NPY)、P物质(SP)水平均低于对照组,差异有统计学意义(P<0.05)。试验组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜无张力疝修补术可以有效降低腹股沟疝患者血清疼痛指标水平和并发症发生率,有助于患者的术后恢复。
Objective:To explore the effect of laparoscopic tension-free hernia repair surgery in the treatment of inguinal hernia patients and its impact on serum pain indicators and complications.Method:A total of 100 patients with inguinal hernia admitted to Nantong Cancer Hospital(South Hospital)from January 2017 to January 2023 were selected by random sampling method.According to random number table method,they were divided into experimental group and control group,50 cases in each group.The control group underwent open tension-free hernia repair surgery,and the experimental group underwent laparoscopic tension-free hernia repair surgery.The perioperative indexes,preoperative and postoperative pain levels,preoperative and postoperative 1 h serum pain indexes and complications were compared between the two groups.Result:The surgery time and hospital stay of the experimental group were shorter than those of the control group,the intraoperative blood loss volume was less than that of the control group,and the anal exhaust time was earlier than that of the control group,the differences were statistically significant(P<0.05).At 1 d after surgery,visual analogue scale(VAS)score of experimental group was lower than that of control group,and the difference was statistically significant(P<0.05).At 1 h after surgery,the levels of norepinephrine(NE),nerve growth factor(NGF),neuropeptide Y(NPY)and substance P(SP)in experimental group were lower than those in control group,and the differences were statistically significant(P<0.05).The incidence of complication of experimental group was lower than that of control group,and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic tension-free hernia repair surgery can effectively reduce the serum pain indicators level and incidence of complications in patients with inguinal hernia,contribute to postoperative recovery of patients.
