摘要
目的探讨磁共振对比增强成像(DCE-MRI)及弥散加权成像(DWI)对乳腺良恶性病变的鉴别诊断价值。方法回顾性分析郑州大学第五附属医院自2020年1月至2023年1月收治的有明确手术病理结果的60例乳腺病变患者的临床及MRI资料,其中良性病变40例、恶性病变20例,且所有患者均行术前DCE-MRI及DWI检查。比较DCE-MRI、DWI相关诊断参数在乳腺良恶性乳腺病变在中的差异,并同时对单一DCE-MRI、DWI诊断及二者联合诊断乳腺疾病的效能进行比较分析。结果在DCE-MRI诊断参数比较图像上,良性乳腺良性病变在形态规则、边缘清晰、强化均匀及时间—信号强度曲线(TIC)分型呈Ⅰ型上百分比均高于恶性乳腺恶性病变(P<0.05)。乳腺良性病变的ADC均值高于乳腺恶性病变(P<0.05)。DCE-MRI联合DWI诊断的准确度均高于单一的DCE-MRI及DWI诊断乳腺良恶性病变的准确度(P<0.05)。结论乳腺良恶性病变鉴别诊断中采取DCE-MRI、DWI检查可取得不错的价值,可根据诊断形态、边缘、强化、ADC值等情况进行鉴别,同时二者联合诊断其诊断效能较高,值得在乳腺良恶性病变诊断及鉴别中应用。
Objective To evaluate the differential value of dynamic contrast-enhancement magnetic resonance imaging(DCE-MRI)and diffusion weighted image(DWI)in benign and malignant breast lesions.Methods A total of 60 cases of the Fifth Affiliated Hospital of Zhengzhou University from January 2020 to January 2023 were studied.All cases were confirmed by operation and pathology,including 40 cases of benign lesions and 20 cases of malignant lesions.All patients underwent DCE-MRI and DWI examination to compare the differences of benign and malignant breast lesions in DCE-MRI and DWI diagnosis parameters,and to compare the efficacy of single DCE-MRI and DWI diagnosis and their combined diagnosis of breast diseases.Results Compared with the diagnostic parameters of DCE-MRI,the percentage of benign breast lesions in regular shape,clear edge,uniform enhancement and time-signal intensity curve(TIC)type I was higher than that of malignant breast lesions(P<0.05).The mean ADC value of benign breast lesions was higher than that of malignant breast lesions(P<0.05).The diagnostic accuracy of DCE-MRI combined with DWI was higher than that of DCE-MRI or DWI alone in the diagnosis of benign and malignant breast lesions(P<0.05).Conclusion The use of DCE-MRI and DWI can achieve good value in the differential diagnosis of benign and malignant breast lesions,which can be differentiated according to the diagnostic morphology,edge,enhancement,ADC value,etc.Meanwhile,the combined diagnosis of the two methods has high diagnostic efficacy,and is worthy of application in the diagnosis and differentiation of benign and malignant breast lesions.
