摘要
目的 评价不同中医疗法防治化疗所致周围神经毒性的干预效果,为临证治疗提供切实参考依据。方法 通过检索中国知网、维普资讯中文期刊服务平台、万方数据知识平台、中国生物医学文献服务系统、PubMed、EMbase、Cochrane Library、Web of Science中关于中医药防治化疗致周围神经毒性的随机对照临床研究,检索时限均从建库至2021年12月,以周围神经毒性发生率作为评价指标,选用Cochrane偏倚风险评估工具进行质量评估,并运用STATA 15.0进行网状荟萃分析。结果 共纳入31篇随机对照试验,涉及3种中医疗法,分析显示与对照组(单纯化疗或加安慰剂)相比,中药口服、中药外用、中药注射剂均能降低化疗致周围神经毒性发生率。根据累积排序概率曲线下面积,由高到低不同疗法排序为中药注射剂、中药口服、中药外用治疗。结论 运用中药口服、中药外用、中药注射剂均能有效降低化疗致周围神经毒性发生率;加用中药口服与中药外用疗法可降低严重周围神经毒性发生率。但因规范化大样本随机对照试验文献数量有限,本研究结论仍需进一步临床研究提供证据支持。
Objective To evaluate the intervention effect of different traditional Chinese medicine(TCM)therapy on prevention and treatment of chemotherapy-induced peripheral neuropathy(CIPN),and to provide reference for clinical treatment.Methods Computer retrieval was conducted to search the China National Knowledge Internet(CNKI),VIP database,Wanfang database,Chinese Biomedical Literature Service System,PubMed,EMbase,Cochrane Library,Web of Science examining for randomized controlled clinical trials of different TCM therapies for the prevention and treatment of CIPN.The search time of the database was from the establishment to December 2021.The incidence of CIPN was as the evaluation index,and STATA15.0 was used for the network meta-analysis after the quality was evaluated by Cochrane risk of bias tool.Results Eligible 31 randomized controlled trialsinvolved 3 TCM trerapy.The results showed that oral administration of TCM,external treatment of TCM,and TCM injection could reduce the incidence of CIPN compared with the control group(single chemotherapy or chemotherapy combined with placebo).Different TCM trerapies,which were ranked by area under curve of cumulative ranking probabilities,the order from high to low was:TCM injection,oral administration of TCM and external treatment of TCM,respectively.Conclusion Oral Chinese Medicine,external treatment of Chinese medicine and TCM injection can effectively reduce the total incidence of CIPN.Due to the limited amount of literature received,clinical studies are still needed to support this.
