摘要
目的探讨某三级综合医院的泌尿道感染病原菌分布及耐药性, 为临床合理使用抗菌药物及院内感染监控提供指导。方法从本院检验系统中筛选2019年1月1日至2021年12月31日泌尿道感染患者的4 748例尿标本培养结果, 回顾性分析其病原菌分布及耐药性。结果 4 748例尿培养标本共分离出1 026株病原菌, 其中革兰阴性菌812株(79.14%), 革兰阳性菌210株(20.47%), 真菌4株(0.39%)。病原菌占比前5位分别是大肠埃希菌610株(59.45%)、屎肠球菌81株(7.89%)、粪肠球菌76株(7.41%)、肺炎克雷伯菌60株(5.85%)、彭氏变形菌51株(4.97%);大肠埃希菌对碳青霉烯类的耐药率<2%, 对阿米卡星、哌拉西林/他唑巴坦的耐药率<10%;粪肠球菌对氨苄西林、呋喃妥因的耐药率<10%, 屎肠道球菌对青霉素类、喹诺酮类、红霉素等的耐药率>90%, 粪肠球菌对青霉素类、呋喃妥因的耐药率<10%, 未检出对万古霉素、利奈唑胺、替加环素耐药的菌株。结论泌尿道感染病原菌以大肠埃希菌为主, 其次为屎肠球菌、粪肠球菌。菌群分布及耐药性监测有利于掌握本地区病原菌的流行病学特点, 可降低和控制细菌耐药进展。
Objective To investigate the distribution and drug resistance of pathogenic bacteria in urinary tract infection in a tertiary general hospital,and provide guidance for rational use of antibiotics and nosocomial infection monitoring.MethodssCulture results of 4748 urine samples of patients with urinary tract infection from January 1,2019 to December 31,2021 were selected from the laboratory system of our hospital,and the distribution and drug resistance of pathogens were retrospectively analyzed.Results A total of 1026 strains of pathogenic bacteria were isolated from 4748 urine culture samples,including 812 gram-negative bacteria(79.14%),210 gram-positive bacteria(20.47%)and 4 fungi(0.39%).The top 5 pathogenic bacteria were 610 strains of escherichia coli(59.45%),81 strains of enterococcus faecium(7.89%),76 strains of enterococcus faecalis(7.41%),60 strains of klebsiella pneumoniae(5.85%)and 51 strains of proteus peni(4.97%).The drug resistance rate of escherichia coli to carbapenems was<2%,and the drug resistance rate to amikacin,piperacillin/tazobactam was<10%.The resistance rate of enterococcus faecalis to ampicillin and furantoin was<10%,the resistance rate of enterococcus faecium to penicillins,quinolones,erythromycin was>90%.The resistance rate of enterococcus faecalis to penicillins and furantoin was less than 10%.No strains resistant to vancomycin,linezolid and tigacycline were detected.Conclusions The main pathogenic bacteria of urinary tract infection were escherichia coli,followed by enterococcus faecium and enerococcus faecalis.Monitoring of bacterial flora distribution and drug resistance is conducive to understanding the epidemiological characteristics of pathogenic bacteria in this area,and can reduce and control the development of bacterial drug resistance.
