摘要
目的探讨子宫颈扩张球囊用于晚期妊娠促宫颈成熟的临床效果。方法选取2018年6月至2020年6月抚州市妇幼保健院妇产科收治的98例晚期妊娠产妇作为研究对象,随机分为对照组与实验组,每组49例。对照组予以催产素促宫颈成熟引产,实验组予以子宫颈扩张球囊联合催产素促宫颈成熟引产。比较两组治疗前后宫颈成熟度、总产程时间、产后不同时间出血量、住院时间及并发症和剖宫产发生情况。结果治疗前,两组Bishop评分比较差异无统计学意义;治疗后,两组Bishop评分均高于治疗前,且实验组高于对照组,差异有统计学意义(P<0.05)。实验组第一、二、三及总产程均短于对照组,差异有统计学意义(P<0.05)。产后2 h,两组出血量均少于产后24 h,产后2、24 h,实验组出血量均少于对照组,住院时间短于对照组,差异有统计学意义(P<0.05)。实验组剖宫产率、并发症发生率均低于对照组,差异有统计学意义(P<0.05)。结论子宫颈扩张球囊可显著提升晚期妊娠产妇宫颈成熟度,加速分娩进程,降低剖宫产率、并发症发生率及产妇产后出血量,有利于产妇产后快速恢复,值得临床推广应用。
Objective To investigate the clinical effect of cervical dilation balloon for cervical ripening in late pregnancy.Methods 98 cases of late-term pregnant women from June 2018 to June 2020 Department of Obstetrics and Gynecology admitted to the Fuzhou Maternal and Child Health Hospital were selected as research subjects,and they were randomly divided into the control group and the experimental group,with 49 cases in each group.The control group was given oxytocin to promote cervical maturity and induce labor,and the experimental group was given cervical dilation balloon combined with oxytocin to promote cervical maturity and induce labor.The cervical maturity,total labor time,postpartum blood loss,hospital stay,complications and cesarean section were compared between the two groups before and after treatment.Results Before treatment,there was no statistically significant difference in Bishop score between the two groups;after treatment,Bishop score of the two groups were higher than before treatment,and the experimental group was higher than the control group,and the differences were statistically significant(P<0.05).The first,second,third and total labor in the experimental group were shorter than those in the control group,the differences were statistically significant(P<0.05).2 h after delivery,the amount of blood loss of two groups were less than those in 24 h after delivery;2 and 24 h after delivery,the amount of blood loss in the experimental group were less than those in the control group,and the length of hospitalization was shorter than that in the control group,and the differences were statistically significant(P<0.05).The cesarean section rate and complication rate in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Cervical dilation balloon can significantly improve the maturity of the cervix,accelerate the process of delivery,reduce the rate of cesarean section,the incidence of complications and the amount of postpartum bleeding,which is conducive to the rapid recovery of postpartum women,and is worthy of clinical promotion and application.
