摘要
目的:探讨柴胡加龙骨牡蛎汤加减联合利培酮治疗精神分裂症患者的临床效果。方法:选取2021年1月—2022年5月在赣州市第三人民医院接受治疗的80例精神分裂症患者进行研究,经随机数字表法分为对照组与研究组,每组40例。对照组采用利培酮治疗;研究组在对照组基础上加用柴胡加龙骨牡蛎汤加减治疗,两组患者均连续治疗12周。比较两组患者临床疗效、精神病性症状严重程度、生活质量、神经营养因子、不良反应发生情况、复发情况。结果:研究组总有效率(92.50%)高于对照组(70.00%),差异有统计学意义(P<0.05)。治疗4、8、12周后,两组简明精神病评定量表(BPRS)评分均较治疗前降低,且研究组均低于对照组,世界卫生组织生命质量量表(WHOQOL-BREF)各维度评分均较治疗前上升,且研究组均高于对照组,差异均有统计学意义(P<0.05)。治疗12周后,两组患者脑源性神经营养因子(BDNF)、神经生长因子(NGF)、神经营养因子3(NT-3)水平均较治疗前提升,且研究组均高于对照组,差异均有统计学意义(P<0.05)。研究组复发率(10.00%)低于对照组(30.00%),差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:针对精神分裂症患者采用柴胡加龙骨牡蛎汤加减联合利培酮治疗不仅能提升临床疗效,改善临床症状与生活质量,还能调节神经营养因子,降低复发率,安全性可靠。
Objective:To investigate the clinical effect of modified Chaihu Plus Longgu Muli Decoction combined with Risperidone in the treatment of schizophrenia.Method:A total of 80 patients with schizophrenia who received treatment in Third People's Hospital of Ganzhou from January 2021 to May 2022 were selected and divided into control group and study group by random number table method,with 40 cases in each group.The control group was treated with Risperidone.The study group was additionally treated with modified Chaihu Plus Longgu Muli Decoction on the basis of the control group,and both groups were treated for 12 weeks.The clinical efficacy,severity of psychotic symptoms,quality of life,neurotrophic factors,occurrence of adverse reactions and recurrence were compared between the two groups.Result:The total effective rate of the study group(92.50%)was higher than that of the control group(70.00%),the difference was statistically significant(P<0.05).After 4,8 and 12 weeks of treatment,the scores of the brief psychiatric rating scale(BPRS)in both groups were lower than those before treatment,and the study group were lower than those in the control group,the scores of all dimensions of the WHO quality of life scale-brief(WHOQOL-BREF)were higher than those before treatment,and the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,the levels of brain-derived neurotrophic factor(BDNF),serum nerve growth factor(NGF)and neurotrophic factor 3(NT-3)in two groups were higher than those before treatment,and the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The recurrence rate in the study group(10.00%)lower than that in the control group(30.00%),the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: For patients with schizophrenia, the use of modified Chaihu Plus Longgu Muli Decoction combined with Risperidone can not only improve clinical efficacy, improve clinical symptoms and quality of life, but also regulate neurotrophic factors, reduce the recurrence rate, safe and reliable.
