摘要
目的探究综合保温干预联合麻醉复苏护理对老年全身麻醉患者苏醒期应激反应及苏醒质量的影响。方法选取2020年1月至12月我院收治的80例接受全身麻醉手术治疗的老年患者为研究对象,按随机数字表法将其分为对照组和观察组,各40例。对照组给予麻醉复苏护理,观察组在对照组基础上给予综合保温干预。比较两组的苏醒质量、应激反应指标及并发症发生情况。结果观察组的完全苏醒时间、自主呼吸恢复时间、意识恢复时间及拔除气管时间短于对照组,差异具有统计学意义(P<0.05)。麻醉前,两组的皮质醇(Cor)、去甲肾上腺素(NE)、肾上腺激素(AD)、促肾上腺皮质激素(ACTH)水平比较,差异无统计学意义(P>0.05);拔管时,观察组的Cor、NE、AD、ACTH水平低于对照组,差异具有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异具有统计学意义(P<0.05)。结论综合保温干预联合麻醉复苏护理可以提高老年全身麻醉患者的苏醒质量,减轻应激反应,安全性高,值得临床推广。
Objective To explore the effects of comprehensive thermal insulation intervention combined with anesthesia resuscitation nursing on stress response in recovery period and recovery quality of elderly patients with general anesthesia.Methods A total of 80 elderly patients who underwent general anesthesia surgery in our hospital from January to December 2020 were selected as the research objects,and the patients were divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was given anesthesia resuscitation nursing,and the observation group was given comprehensive thermal insulation intervention on the basis of the control group.The quality of recovery,stress response indexes and complications were compared between the two groups.Results The complete recovery time,spontaneous breathing recovery time,consciousness recovery time and extubation time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).Before anesthesia,there were no significant differences in the levels of cortisol(Cor),norepinephrine(NE),adrenal hormone(AD)and adrenocorticotropic hormone(ACTH)between the two groups(P>0.05);at extubation,the levels of Cor,NE,AD and ACTH in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Comprehensive thermal insulation intervention combined with anesthesia resuscitation nursing can improve the recovery quality of elderly patients with general anesthesia,reduce stress response,and has high safety,which is worthy of clinical promotion.
