内耳磁共振钆造影与3D-FLAIR平扫对梅尼埃病患者前庭积水的诊断价值比较 被引量:1

摘要 目的探讨内耳磁共振(MRI)钆造影与三维液体衰减反转恢复(3D-FLAIR)平扫对梅尼埃病患者前庭积水的诊断价值。方法回顾性分析2020年6月至2022年10月在乐山市人民医院就诊的44例梅尼埃病患者的临床资料,比较内耳MRI钆造影及3D-FLAIR平扫诊断前庭积水的一致性。结果内耳MRI钆造影对梅尼埃患者前庭积水的诊断敏感度(95.34%)较3D-FLAIR平扫(88.63%)更高;内耳MRI钆造影与3D-FLAIR平扫诊断梅尼埃病前庭积水的一致性中等(Kappa=0.425,P<0.001),而这两种方法对前庭积水程度的诊断一致性较差(Kappa=0.372,P<0.001)。结论内耳MRI 3D-FLAIR平扫作为一种无创的检查方式,能在一定程度上判断内耳前庭积水情况,可为梅尼埃病的诊断提供参考;而钆造影对该病的诊断价值更高。 Objective To investigate the value of inner ear magnetic resonance imaging(MRI)gadolinium angiography and three dimensional fluid attenuated inversion recovery(3D-FLAIR)plain scan in the diagnosis of vestibular hydrops in patients with Meniere's disease.Methods The clinical data of 44 patients with Meniere's disease in the People's Hospital of Leshan from June 2020 to October 2022 were retrospectively analyzed.The consistency of inner ear MRI gadolinium angiography and 3D-FLAIR plain scan in the diagnosis of vestibular hydrops was compared.Results MRI gadolinium angiography of inner ear(95.34%)was more sensitive than 3D-FLAIR plain scan(88.63%)in the diagnosis of vestibular hydrops in Meniere's patients.The consistency between MRI gadolinium angiography and 3D-FLAIR plain scan in the diagnosis of vestibular hydrops was moderate(Kappa value=0.425,P<0.001),but the consistency of the two methods in the diagnosis of the degree of vestibular hydrops was poor(Kappa value=0.372,P<0.001).Conclusion Inner ear MRI 3D-FLAIR plain scan,as a noninvasive examination method,can detect vestibular hydrops to a certain extent,which can provide certain reference for the diagnosis of Meniere's disease.Gadolinium angiography of inner ear has higher value in the diagnosis of Meniere's disease.
