摘要
目的:探讨Pierre Robin序列征病儿术前营养状况及相关危险因素。方法:收集2021年6月至2022年12月在南京医科大学附属儿童医院诊治的120例Pierre Robin序列征病儿术前临床资料及营养风险筛查和营养评定结果,根据营养状况将120例病儿分为营养不良组80例及营养正常组40例,采用Pearson卡方检验及二元Logistic回归分析探讨Pierre Robin序列征病儿营养不良的危险因素。结果:120例病儿中高营养风险者77例,发生率64.2%,营养不良者80例,占总病例66.7%。单因素分析发现,营养不良组病儿在出生体质量偏低、宫内发育迟缓(IUGR)、合并腭裂或其他先天畸形等方面与营养正常组病儿间具有统计学差异(P <0.05)。多因素Logistic回归分析显示存在宫内发育迟缓(IUGR)、腭裂是Pierre Robin序列征病儿发生营养不良的独立危险因素,其比值比(OR)分别为7.04(95%CI:1.543~32.126,P <0.05)、2.68(95%CI:1.180~6.088,P <0.05)。结论:Pierre Robin序列征病儿营养不良发生率较高,对存在宫内发育迟缓(IUGR)、腭裂的Pierre Robin序列征病儿应尽早干预,改善病儿营养状况,提高手术效果,促进术后恢复。
Objective:To investigate the nutritional status of infants with Pierre Robin sequence,and to analyze the related risk factors on their nutritional status.Methods:The clinical findings from 120 inpatients with Pierre Robin sequence treated in Nanjing Medical University Affiliated Children's Hospital were retrospectively analyzed,who were recruited form June 2021 to December 2022.Patients were divided into either the malnutrition group(n = 80) or the control group(n = 40) according to their nutritional status.Chi-square test and Logistic regression were performed to analyze the risk factors of malnutrition in infants with Pierre Robin sequence.Results:The incidence rates of malnutrition infants were 66.7%(80/120),the incidence rates of high nutrition risks were 64.2%(77/120).Univariate analysis showed that birth weight,IUGR,cleft palate and other congenital malformations were related to the malnutrition in infants with Pierre Robin sequence,and the differences were statistically significant(P<0.05).Furthermore,binary Logistic regression analysis identified that patients with IUGR(OR = 7.04,95%CI = 1.543 ~ 32.126) and cleft palate(OR=2.68 95%CI = 1.180 ~ 6.088) were independent risk factors for malnutrition in infants with Pierre Robin sequence.Conclusion:The incidence of malnutrition in infants with Pierre Robin sequence is relatively high.IUGR and cleft palate are the main factors affecting the nutritional status of patients.Clinical intervention measures can be taken to improve the nutritional status of patients and ensure the effect of operation.
