摘要
目的探讨浙江省全科医生阅读及应用临床实践指南的情况,了解其相关需求及意见。方法对浙江大学医学院附属第一医院全科联盟单位内的全科医生进行问卷调查,内容包括全科医生的基本情况、临床实践指南(简称指南)阅读习惯、指南应用情况、对指南的意见和培训需求等。结果受访者中,84.48%的全科医生阅读过指南,94.22%认为自己需要进行相关培训指导。在读过指南的全科医生中,92.31%阅读过针对基层的指南,100%认为指南对临床工作有帮助,99.57%参照指南开展工作。多种因素对全科医生指南阅读习惯和实践产生影响。高职称(P=0.005)、综合性医院(P=0.009)全科医生阅读指南的比例更高。高职称(P=0.035)认为基层指南实用(P<0.001)、有过指南与工作经验不符合(P=0.049)的全科医生更希望得到指南相关培训。结论建议进一步扩展指南在基层全科医生中的推广宣传力度,完善指南制定管理流程,在编写过程中加大对基层指南的倾斜,深化全科医生群体指南培训工作,以此进一步拓展全科医生对指南的了解及应用。
Objective To investigate the reading and application of clinical practice guidelines by general practitioners(GPs)in Zhejiang Province,and understand their related needs and opinions.Methods A questionnaire survey was conducted among GPs in the General Practice Alliance of the First Affiliated Hospital of Zhejiang University School of Medicine.The content included GPs'basic information,reading habits,application of guidelines,suggestions and training needs.Results Among the respondents,84.48%of the GPs had read the guideline,and 94.22%thought they needed guideline-related training guidance.Among the GPs who had read the guideline,92.31%read the guideline for primary care,100%believed that the guideline was helpful to clinical work,99.57%referred to the guideline for work.A variety of factors influenced GPs'guideline reading habits and practice.Higher professional titles(P=0.005)and general hospitals(P=0.009)had a higher proportion of GPs reading the guidelines.GPs with high professional titles(P=0.035)who felt that the primary guidelines were practical(P<0.001),and those who had experience inconsistence with the guidelines and work experience(P=0.049),were more likely to receive training related to the guidelines.Conclusion In the management of guidelines,we can further expand the promotion and publicity of guidelines among primary care GPs,continue to improve the management process of guidelines,increase the inclination to primary care guidelines in the preparation process,and deepen the training of guidelines for GPs,so as to further expand the understanding and application of guidelines among GPs.
