外周血TAP检测在淋巴瘤及骨髓瘤的应用价值分析

摘要 目的 探讨外周血异常糖链糖蛋白(TAP)检测在淋巴瘤及骨髓瘤的应用价值。方法 随机选取2019年11月-2022年8月医院血液内科收治的淋巴瘤和多发性骨髓瘤患者各58例,分别设为淋巴瘤组和多发性骨髓瘤组,另招募健康志愿者40例,设为健康对照组。抽取患者空腹状态下外周静脉血,采用凝集法测定外周血TAP水平。比较三组外周血TAP水平,受试者工作特征(ROC)曲线下面积(AUC)评价外周血TAP检测诊断淋巴瘤和多发性骨髓瘤以及鉴别淋巴瘤骨髓浸润和多发性骨髓瘤的价值,并分析外周血TAP水平与淋巴瘤骨髓浸润以及多发性骨髓瘤免疫分型的关系。结果 与健康对照组比较,淋巴瘤组和多发性骨髓瘤组外周血TAP水平显著升高(P<0.05),而淋巴瘤组和多发性骨髓瘤组外周血TAP水平比较,差异无统计学意义(P>0.05)。外周血TAP检测诊断淋巴瘤的敏感度、特异度、AUC分别为64.38%、67.52%、0.596,诊断多发性骨髓瘤的敏感度、特异度、AUC分别为62.12%、64.89%、0.511。骨髓浸润的淋巴瘤患者外周血TAP水平显著高于骨髓未浸润者(P<0.05),淋巴瘤患者外周血TAP水平与骨髓浸润情况呈显著正相关(r=0.432,P<0.05)。不同免疫分型多发性骨髓瘤患者外周血TAP水平比较,差异无统计学意义(P>0.05),多发性骨髓瘤患者外周血TAP水平与免疫分型无显著相关性(r=0.214,P=0.675)。外周血TAP检测鉴别淋巴瘤骨髓浸润和多发性骨髓瘤的敏感度、特异度、AUC分别为69.48%、73.56%、0.621。结论 淋巴瘤和骨髓瘤患者外周血TAP水平显著升高,外周血TAP可作为诊断淋巴瘤和骨髓瘤的辅助指标,并可对淋巴瘤骨髓浸润提供较为可靠的依据,具有临床应用价值。 Objective To explore the application value of abnormal glycoprotein(TAP)detection in peripheral blood in lymphoma and myeloma.Methods 58 patients with lymphoma and multiple myeloma admitted to the hospital department of Hematology from November 2019 to August 2022 were randomly selected as the lymphoma group and multiple myeloma group,and 40 healthy volunteers were recruited as the healthy control group.Peripheral venous blood was drawn from the fasting state and TAP levels were determined by agglutination.Compare three groups of peripheral blood TAP levels,subject working characteristics(ROC)curve area(AU C)evaluation of peripheral blood TAP detection diagnosis of lymphoma and multiple myeloma and differential lymphoma bone marrow infiltration and multiple myeloma value,and analyze the peripheral blood TAP level and lymphoma bone marrow infiltration and multiple myeloma immunotyping relationship.Results Compared with the healthy controls,the peripheral TAP levels were significantly higher in the lymphoma and multiple myeloma groups(P<0.05),while the peripheral TAP levels in the lymphoma and multiple myeloma groups were not significant(P>0.05).The sensitivity,specificity,and AU C were 64.38%,67.52%,and 0.596,respectively,and the sensitivity,specificity,and AU C for diagnostic multiple myeloma were 62.12%,64.89%,and 0.511,respectively.Peripheral TAP levels in peripheral blood were significantly higher in patients with bone marrow infiltration than in those with no bone marrow infiltration(P<0.05),and peripheral TAP levels were significantly positively associated with bone marrow infiltration in patients with lymphoma(r=0.432,P<0.05).TAP levels in patients with different immune subtypes(P>0.05),and no significant correlation between TAP levels in patients with multiple myeloma(r=0.214,P=0.675).The sensitivity,specificity of peripheral blood TAP detection,and AU C were 69.48%,73.56%,and 0.621,respectively.Conclusion Peripheral blood TAP level is significantly increased in lymphoma and myeloma patients,and peripheral blood TAP can serve as an auxiliary index for the diagnosis of lymphoma and myeloma,and can provide a more reliable basis for lymphoma bone marrow infiltration,and has clinical application value.
