摘要
目的探讨术后布地奈德雾化联合两性霉素B冲洗在嗜酸性粒细胞鼻窦炎中的应用价值。方法选取2019年2月-2022年2月海安市中医院收治的84例嗜酸性粒细胞鼻窦炎术后患者,依照随机数表法分为研究组(n=42例)和对照组(n=42例),对照组采用布地奈德雾化治疗,研究组在布地奈德雾化治疗的同时给予两性霉素B冲洗治疗。比较两组患者鼻部症状视觉模拟疼痛评分(VAS),内镜Lund-Kennedy评分,临床疗效,T淋巴细胞亚群、嗜酸性粒细胞计数、血清超敏C反应蛋白(hsCRP)水平及复发情况。结果治疗后,两组患者鼻部症状VAS评分、内镜Lund-Kennedy评分、嗜酸性粒细胞计数和血清hsCRP水平均降低(P均<0.05),且研究组低于对照组(P<0.05);研究组总有效率高于对照组(P<0.05);两组患者CD3+、CD4+、CD4+/CD8+水平均升高(P<0.05),且研究组高于对照组(P<0.05);随访6个月,研究组复发率低于对照组(P<0.05)。结论布地奈德雾化联合两性霉素B冲洗用于治疗嗜酸性粒细胞鼻窦炎术后患者可以改善临床症状,疗效确切,同时可以增强免疫功能,降低复发率。
Objective To explore application value of budesonide atomization combined with amphotericin B irrigation in postoperative eosinusitis.Methods 84 patients with eosinophils inophilic sinusitis after surgery from February 2019 to February 2022 in the hospital.The patientswere selected and randomly divided into study group(n=42)and control group(n=42).The control group was treated with budesonide atomization,while the study group was treated with amphotericin B douche along with budesonide atomization.The visual analog pain score(VAS)of nasal symptoms,Lund Kennedy score of endoscopy,clinical efficacy,T-lymphocyte subsets,eosinophils inophil count,serum high-sensitivity C-reactive protein(hsCRP)level and recurrence were compared in both groups.Results After thetreatment,the nasal symptom VAS score,endoscopic Lund Kennedy score,eosinophil count,and serum hsCRP level in both groups of patients were decreased(P<0.05),and the study group had a lower score(P<0.05).The total effective rate of the research group was higher than that of control group(P<0.05).The levels of CD3+,CD4+and CD4+/CD8+in both groups were increased(P<0.05),and the levels in the study group were even higher(P<0.05).Following up for 6 months,the recurrence rate in study group was lower than that in control group(P<0.05).Conclusion The application of budesonide atomization combined with amphotericin B washing in the treatment of the patients with eosinophils inophilic nasosinusitis after surgery can improve the clinical symptoms and have a definite effect,and it can enhance the immune function and reduce the recurrence rate.
