摘要
目的 探讨戊酸雌二醇片/雌二醇环丙孕酮片联合屈螺酮炔雌醇片治疗异常子宫出血(AUB)的效果。方法 选取2022年1—10月潜江市中心医院收治的AUB患者106例,按随机数表法分为两组,每组各53例,对照组口服屈螺酮炔雌醇片治疗,在此基础上观察组联合口服戊酸雌二醇片/雌二醇环丙孕酮片治疗,治疗3个月后评估两组患者治疗前后血红蛋白变化和子宫内膜厚度的变化以及治疗前后碱性成纤维细胞生长因子(bFGF)、血管内皮生长因子(VEGF)水平变化、不良反应等。结果 两组患者治疗后,观察组血红蛋白水平明显高于对照组,差异有统计学意义(P<0.05),观察组子宫内膜厚度小于对照组,差异有统计学意义(P<0.05);两组患者血红蛋白水平治疗后均高于治疗前,子宫内膜厚度治疗后均小于治疗前,差异有统计学意义(P<0.05)。观察组总有效率为96.23%,高于对照组的81.13%(P<0.05);治疗后观察组血清bFGF、VEGF水平高于对照组(P<0.05);观察组不良反应总发生率为16.98%,与对照组的11.32%相比,差异无统计学意义(P>0.05)。结论 戊酸雌二醇片/雌二醇环丙孕酮片联合屈螺酮炔雌醇片能增加AUB患者的血红蛋白水平,降低子宫内膜厚度,并升高患者血清bFGF、VEGF水平,提升临床疗效,且不良反应少。
Objective To investigate the effect of estradiol valerate tablet/estradiol cyproterone tablet combined with drospirosterone edylestradiol tablet in the treatment of patients with abnormal uterine bleeding.Methods A total of 106 patients with abnormal uterine bleeding admitted to and treated in Qianjiang Central Hospital from January to October 2022 were selected,and they were divided into the control group(n=53)and the observation group(n=53)according to the random number table method.The control group was orally administrated with drospirenone ethinyl estradiol tablets,while the observation group was orally administrated with estradiol valerate tablets/estradiol cyproterone tablets on the basis of the treatment in the control group.After 3 months of treatment,the changes of hemoglobin,endometrial thickness,basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)before and after treatment were evaluated in both groups growth factor VEGF level changes,adverse reactions,etc.Results After treatment,the hemoglobin levels in the observation group were remarkably higher than those in the control group,with statistically significant differences(P<0.05),and the endometrial thickness in the observation group was smaller than that in the control group,with statistically significant difference(P<0.05).After treatment,the hemoglobin levels of both groups of patients were higher than those before treatment,and the endometrial thickness were smaller than those before treatment,with statistically significant differences(P<0.05).The overall response rate in the observation group was 96.23%,higher than 81.13%in the control group(P<0.05);After treatment,the levels of serum bFGF and VEGF in the observation group were higher than those in the control group(P<0.05).The total incidence of ADRs in the obs ervation group was 16.98%,and that in the control group was 11.32%,without statistically significant difference(P>0.05).Conclusion Estradiol valerate tablets/estradiol cyproterone tablets combined with drospirenone ethinyl estradiol tablets can increase the hemoglobin level, reduce the endometrial thickness, and increase the levels of serum bFGF and VEGF in patients with abnormal uterine bleeding. In addition, it improves the clinical efficacy, with fewer ADRs.
