摘要
目的 比较内镜下单侧双通道技术治疗腰椎椎管狭窄症放置与未放置引流管术后的临床疗效及安全性。方法 前瞻性分析2021年1月—2023年4月阿克苏地区第一人民医院的87例腰椎椎管狭窄症患者资料。按术后是否放置引流管方式不同分为两组,放置引流组43例,未放置引流组44例。分析比较两组术中以及术后各个时间阶段的相应指标。结果 未放置引流组发生术后并发症发生率为9.09%,放置引流组并发症发生率为6.98%,差异无统计学意义(χ^(2)=0.001,P>0.05)。未放置引流组在术后1 d疼痛视觉模拟评分(4.56±0.63)分优于放置引流组,差异有统计学意义(t=4.519,P<0.05),两组剩余时间段疼痛视觉模拟评分及所有时间段Oswestry功能障碍指数评定评分比较,差异无统计学意义(P>0.05)。结论 行单侧双通道内镜技术治疗腰椎管狭窄症中不放置引流管未引发术后并发症,且可减少术后疼痛、住院时间,促进患者早期下地活动,故在行内镜下单侧双通道技术治疗腰椎间盘突出症中可以不放置引流管。
Objective To compare the clinical efficacy and safety of endoscopic unilateral dual-channel technique in the treatment of lumbar spinal stenosis with and without drain placement after surgery.Methods Prospectively analyze the data of 87 patients with lumbar spinal stenosis in Aksu First People´s Hospital from January 2021 to April 2023.They were divided into two groups according to whether or not the drainage tube was placed after surgery,43 cases in the group with drainage placed and 44 cases in the group without drainage placed.Comparing the corresponding in⁃dexes of the two groups during the operation and at various stages of the postoperative period.Results The incidence of postoperative complications was 9.09%in the group without drainage placement,and 6.98%in the group with drain⁃age placement,and the difference was not statistically significant(χ^(2)=0.001,P>0.05).The Visual Analogue Score(4.56±0.63)points in the group without placement of drainage was better than that in the group with placement of drainage on the 1 day after surgery,and the difference was statistically significant(t=4.519,P<0.05).There was no sta⁃tistically significant difference in the Visual Analogue Score of pain for the remaining time period and Oswestry Dis⁃ability Index for all time periods between the two groups(P>0.05).Conclusion Not placing a drainage tube in the treatment of lumbar spinal stenosis by unilateral dual-channel endoscopic technique did not lead to postoperative complications,and it can reduce postoperative pain and hospitalization time,and promote early ground mobility for patients,so it is possible to treat lumbar disc herniation without placing a drainage tube in the treatment of lumbar spi⁃nal disc herniation by endoscopic unilateral dual-channel technique.
