摘要
目的探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)患者伴血小板减少与调节性T细胞(Treg)比例及血浆IL-10和TGF-β水平的相关性。方法选取SLE伴血小板减少患者20例(A组)、无血小板减少患者47例(B组)、健康对照组13例(C组),流式细胞术分析外周血单个核细胞(PBMC)中的淋巴细胞亚群及Treg占CD4^(+)T细胞的比例,同时应用酶联免疫吸附法(ELISA)分别检测血浆中IL-10和TGF-β1的水平,分析淋巴细胞亚群、Treg、IL-10、TGF-β1与血小板减少的关系及相关性。结果①A组的Treg比例低于B组和C组(P<0.05),IL-10、TGF-β1、淋巴细胞计数、总T淋巴细胞计数、CD4^(+)T淋巴细胞计数低于B组,组间差异有统计学意义(P<0.05);②Spearman等级相关分析显示,血小板计数与Treg、IL-10、TGF-β1、淋巴细胞计数、总T淋巴细胞计数、CD4^(+)T淋巴细胞计数呈正相关(P<0.05)。结论SLE伴血小板减少患者中Treg细胞占CD4^(+)T细胞的比例降低,Treg的数量、比例和功能异常可能在SLE血小板减少的发病机制中发挥作用。
To investigate the correlation of thrombocytopenia with the proportion of regulatory T cells(Treg),plasma IL-10 and TGF-βlevel in patients with systemic lupus erythematosus(SLE),total of 20 SLE patients with thrombocytopenia(group A),47 patients without thrombocytopenia(group B)and 13 healthy controls(group C)were selected.The lymphocyte subsets and the proportion of Treg in CD4^(+)T cells in peripheral blood mononuclear cells were analyzed by flow cytometry,and the level of IL-10 and TGF-β1 in plasma were detected by ELISA.The relationship and correlation of lymphocyte subsets,Treg,IL-10,TGF-β1 with thrombocytopenia were analyzed.Data showed that the proportion of Treg in group A was lower than that in group B and C.Furthermore,IL-10,TGF-β1,lymphocyte count,total T lymphocyte count and CD4^(+)T lymphocyte count in group A were all lower than those in group B.Spearman correlation analysis showed that platelet count was positively correlated with Treg,IL-10,TGF-β1,lymphocyte count,total T lymphocyte count and CD4^(+)T lymphocyte count.In conclusion,the proportion of Treg in CD4^(+)T cells is decreased in SLE patients with thrombocytopenia.The abnormalities in the number,proportion and function of Treg may play an important role in the pathogenesis of thrombocytopenia in SLE.
