摘要
目的分析不同年龄段老年高血压并发髋部骨折患者同步监测的动态血压和动态心电图变化情况,评估心脑血管事件相关风险。方法选取2018年7月至2022年1月在四川省骨科医院住院的65岁及以上老年高血压并发髋部骨折患者162例,分为低龄老年组(65~79岁)84例和高龄老年组(≥80岁)78例,对每位患者行动态心电图及动态血压同步监测,并对其结果进行分析。结果两组患者平均收缩压增高,两组间比较差异无统计学意义(P>0.05);低龄老年组平均舒张压在24 h、日间及夜间均高于高龄老年组(P<0.05);低龄老年组出现平均脉压差大、血压节律紊乱、低心率变异性、频发早搏、阵发性室上速及无症状心肌缺血人数均少于高龄老年组(P<0.05);低龄老年组24 h平均心率慢于高龄老年组(P<0.05)。结论高龄老年组患者血压节律紊乱更突出、心律失常发生率更高,需动态监测血压及心电图,及时发现异常并进行针对性调整,从而积极预防心脑血管事件的发生。
Objective To investigate the changes of ambulatory blood pressure and ambulatory electrocardiogram(ECG)monitored synchronously in different age groups of elderly patients with hypertension complicated with hip fracture,and to assess the risk of cardiovascular and cerebrovascular events.Methods A total of 162 elderly patients aged 65 and above with hypertension complicated with hip fracture who were admitted to Sichuan Orthopedics Hospital from July 2018 to January 2022 were selected.They were divided into the younger elderly group(65-79 years old,n=84)and the older elderly group(≥80 years old,n=78).Ambulatory ECG and ambulatory blood pressure were monitored synchronously in each patient,and the results were analyzed.Results The mean systolic blood pressure increased in both groups,and there was no statistically significant difference between the two groups(P>0.05).The mean diastolic blood pressure(24 h,day,night)in the younger elderly group was higher than that in the older elderly group(P<0.05).The incidence of large difference in mean pulse pressure,blood pressure rhythm disorder,low heart rate variability,frequent premature beats,paroxymal supraventricular tachycardia and silent myocardial ischemia in the younger elderly group were less than those in the older elderly group(P<0.05).The 24 h mean heart rate in the younger elderly group was slower than that in the older elderly group(P<0.05).Conclusion In the older elderly group,the incidence of blood pressure rhythm disorder and arrhythmia are higher,which requires synchronous monitoring of ambulatory blood pressure and ambulatory ECG to detect abnormalities in time and make targeted adjustments to actively prevent the occurrence of cardiovascular and cerebrovascular events.
