摘要
目的探讨腹部CT血管成像(CTA)中影响对比剂到达阈值时间的相关因素,实现个体化腹部CTA扫描延迟时间,减少辐射剂量。资料与方法前瞻性收集2019年12月—2021年6月于大连医科大学附属第一医院行全腹部CTA检查的310例患者,记录性别(男1女2)、年龄、身高、体重、腰围。监测序列采用实时血流检测自动触发技术,触发阈值180 Hu,设置肝门水平腹主动脉作为监测层面,从对比剂注射开始,延迟10 s进行监测,每间隔1 s监测扫描一次,记录每位患者的自动触发所需时间(10 s+监测时间)及辐射剂量。将患者随机分为A组200例和B组110例,A组触发时间与患者性别、年龄、身高、体重、腰围、辐射剂量采用相关性检验、多重线性回归法进行分析,获得腹部CTA对比剂延迟时间计算公式。应用A组获得的公式计算B组触发时间,比较B组实际时间与计算时间的差异。结果A组自动触发时间14~30 s,平均(20.97±3.60)s。触发时间与性别呈负相关(r=-0.555,P<0.05),与年龄、身高、体重、腰围及辐射剂量呈正相关(r=0.208、0.578、0.602、0.485、0.997,均P<0.05)。自动触发时间=-7.479-1.819×性别+0.055×年龄+0.098×身高+0.061×体重+0.075×腰围(R^(2)=0.511,P<0.05)。B组自动触发时间14~31 s,平均(21.15±3.37)s,与公式计算时间(20.92±2.62)s比较,差异无统计学意义(t=0.962,P>0.05)。结论采用定量定速对比剂注射方案行腹部CTA时,自动触发所需时间受患者性别、年龄、身高、体重、腰围影响,其中性别、身高和体重是主要影响因素,可根据多重线性回归方程个体化设置触发扫描的延迟时间,减少监测次数和辐射剂量。
Purpose To analyze the factors that influence the contrast-rise time in bolus-tracking scans in abdominal CT angiography(CTA)after contrast agent injection,in order to adjust the delay time individually to further reduce radiation dose.Materials and Methods A total of 310 patients who underwent abdominal CTA from December 2019 to June 2021 in the First Affiliated Hospital of Dalian Medical University were prospectively collected.Parameters including gender(male 1,female 2),age,height,weight and waist circumference(WC)were recorded.The monitoring scan sequence used the bolus tracking technique to automatically trigger CTA,trigger threshold was 180 Hu.The abdominal aorta at the level of porta hepatis was used as the monitoring level,and the monitoring scans started with a fixed 10 s delay after the contrast agent was injected,with 1 s scan interval.The automatic trigger time(10 s+monitoring scan time)and radiation dose of each patient were recorded.The patients were randomly divided into group A(200 cases)and group B(110 cases).The correlation and multiple linear regression analysis were performed to analyze the relationship between the trigger time and gender,age,height,weight,WC and radiation dose in group A.The delay time formula was used to calculate the trigger time in group B,and the difference between real time and calculated time was compared.Results The automatic trigger time in group A was 14-30 s,with an average of(20.97±3.60)s,and was significantly correlated with gender,age,height,weight,WC and radiation dose(r=-0.555,0.208,0.578,0.602,0.485,0.997,all P<0.05).The multiple linear regression analysis indicated that automatic trigger time=-7.479-1.819×gender+0.055×age+0.098×height+0.061×weight+0.075×WC(R^(2)=0.511,P<0.05).The automatic trigger time in group B was 14-31 s,with an average of(21.15±3.37)s.There was no significant difference between real time and calculated time[(20.92±2.62)s,t=0.962,P>0.05].Conclusion The delay time in the bolus-tracking scans in abdominal CTA with a fixed-contrast dose and flow rate injection scheme is affected by gender,age,height,weight,and WC of patients.Among them,gender,height and weight are the main factors.The delay time may be individually extended according to the multiple linear regression equation,which reduces the monitoring scan times and radiation dose.
