摘要
目的:研究乳腺X射线摄影与B超联合鉴别诊断乳腺肿瘤的效能。方法:选取2020年3月—2023年3月无锡市人民医院收治的103例乳腺肿瘤患者开展试验。对所有受试者均开展B超检查,同时分别实施全视野数字乳腺X射线摄影(full-field digital mammography,FFDM)。将病理检查结果视作金标准,分析乳腺X光机与B超联合应用于乳腺肿瘤鉴别诊断中的效能。此外,对比FFDM与B超联合对不同直径乳腺恶性肿瘤的检出率。结果:病理检查结果显示,恶性肿瘤71例,良性肿瘤32例。FFDM检出恶性肿瘤63例,其中6例误诊,8例漏诊;B超检出恶性肿瘤58例,其中8例误诊,13例漏诊。将病理检查结果视作金标准,FFDM联合B超诊断乳腺恶性肿瘤的灵敏度、特异度及准确率分别为98.59%、96.88%、98.06%,均高于单纯FFDM检查的88.73%、81.25%、86.41%与单纯B超检查的81.69%、75.00%、79.61%,差异有统计学意义(P<0.05)。FFDM联合B超对<2 cm乳腺恶性肿瘤的检出率为97.14%,高于单纯FFDM检查的82.86%与单纯B超检查的77.14%,差异有统计学意义(P<0.05)。结论:FFDM与B超联合诊断乳腺肿瘤的效能较佳,且能够实现对直径<2 c m的微小病灶乳腺癌的诊断,具有较高的临床应用价值,值得推广应用。
Objective To study the combined application efficacy of mammography and B-ultrasound in the differential diagnosis of breast tumors.Methods A total of 103 patients with breast tumor admitted to the Wuxi People's Hospital from March 2020 to March 2023 were selected to carry out the experiment.All subjects underwent B-ultrasound and full-field digital mammography(FFDM).The results of pathological examination were regarded as the gold standard to analyze the efficiency of combined application of FFDM and B-ultrasound in the differential diagnosis of breast tumors.In addition,the detection rate of breast malignant tumors with different diameters was compared between FFDM and B-ultrasound.Results Pathological examination showed that 71 cases of malignant tumor and 32 cases of benign tumor.63 cases of malignant tumor were detected by FFDM,of which 6 cases were misdiagnosed and 8 cases were missed;B-ultrasound detected 58 cases of malignant tumor,of which 8 cases were misdiagnosed and 13 cases were missed.The sensitivity,specificity and accuracy of FFDM combined with B-ultrasound in the diagnosis of breast malignant tumor were 98.59%,96.88%and 98.06%,respectively.They were higher than 88.73%,81.25%,86.41%for FFDM alone and 81.69%,75.00%,79.61%for B-ultrasound alone,the difference were statistically significant(P<0.05).The detection rate of FFDM combined with B-ultrasound for breast malignant tumor<2 cm was 97.14%,higher than that of FFDM alone and B-ultrasound alone 82.86%and 77.14%,the difference was statistically significant(P<0.05).Conclusion The combination of FFDM and B-ultrasound has better efficacy in diagnosing breast tumors,and can realize the diagnosis of small lesions with a diameter of less than 2cm breast cancer.It has high clinical application value and is worth popularizing.
